摘 要:目的探讨超声造影定量分析评估移植肝急性排异的可行性,筛选与移植肝急性排异相关的造影定量参数。方法穿刺病理证实移植肝急性排异患者16例为排异组,移植肝无并发症患者10例为对照组。所有患者均进行超声造影检查,并采集2min的动态影像供脱机分析。超声造影后,计算移植肝内血管渡越时间。超声造影数据脱机分析,获得肝实质相应时间-强度曲线,比较各灌注参数在移植肝正常组与急性排异组的差别。结果移植肝急性排异时,肝动脉-肝静脉渡越时间、门静脉-肝静脉渡越时间分别为(6.12±2.53)s、(3.00±2.62)s,明显短于移植肝正常组相应时间(14.01±3.31)s、(9.60±3.51)s(P〈0.05);肝实质的造影剂达峰时间延迟为(121.06±31.13)s,与移植肝正常组(64.87±33.92)s比较,差异有统计学意义(P〈0.05)。结论超声造影定量曲线对评价移植肝急性排异有一定价值。Objective To investigate the feasibility of contrast-enhanced ultrasonography (CEUS) in the evaluation of acute rejection of transplanted liver and screening of quantitative parameters. Methods Twenty-six patients with transplanted liver were recruited in the study, including 16 patients with acute rejection in transplanted liver as the study group, 10 patients without any complications in graft as the control group. All patients were examined with CEUS and the dynamic images of CEUS (at least 2 minute) were analyzed off line. Regions of interest were drawn in the liver parenchyma to get time-intensity curve and quantitative parameters. Intrahepatie transit time was also calculated. All parameters were compared between the two groups. Results In group with graft acute rejection, the hepatic transit time including hepatic artery-hepatic vein transit time (HA-HVTT) and portal vein-hepatic vein transit time(PV-HVTT) were (6.12 ±2.53)s and (3.00 ±2.62)s, respectively, and significantly shorter than those in group without acute rejection [(14.01 ± 3.31)s and (9.60±3.51)s, P 〈0.05]. The time to peak(TTP) of liver parenchyma was prolonged to (121.06 ± 31.13)s as the acute rejection occurred, while the according time was (64.87 ±33.92)s in group without rejection, which showed a statistical difference between the two groups ( P 〈0.05). Conclusions Quantitative analysis of CEUS can provide information of rejection of transplanted liver.
