医院住院药房中存在的退药问题分析及改进对策

摘　　要：目的探讨医院住院药房中存在的退药问题并制定有效的改进对策,以提高合理用药水平。方法以2014年2月—2015年2月采用在该院住院药房退药的1000例患者作为研究对象,收集退药资料,总结在医院药房中存在的退药原因,并制定相应的改进对策。结果主要药房退药的原因有医嘱调整、处方输入错误、患者死亡、患者出院、药物不良反应、患者或患者家属拒绝用药等,通过制定对应的改进对策,可改善住院药房的合理用药。结论对医院住院药房中存在的退药问题进行调查分析,并制定有效的改进对策,可以提高合理用药水平,减少住院药房的退药情况,值得深入开展。Objective To investigate the problem of drug withdrawal in hospital pharmacy presence and develop an effective improvement measures to enhance the level of rational drug use. Methods February 2014 to February 2015 in our hospital pharmacy using drug withdrawal of 1,000 patients for the study, data were collected drug withdrawal, drug withdrawal sum-marize the reasons that exist in hospital pharmacies, and to develop appropriate improvement measures. Results The main reasons pharmacy prescription drug withdrawal adjust, prescription input error, the patient died, the patient was discharged, adverse drug reactions, patients or family members of patients refuse medication, etc., through the development of corre-sponding improvement measures, can improve the rational use of drugs in hospital pharmacies. Conclusion The problem of drug withdrawal in hospital pharmacy presence of investigation and analysis, and develop effective improvement measures, can improve the level of rational use of drugs, reduce drug withdrawal in hospital pharmacy, the worth depth.
