连续性护理模式在全髋关节置换术患者中的构建及应用

摘　　要：目的探讨全髋关节置换术患者连续性护理模式的构建方式以及应用价值。方法以入院病例号为编号,根据随机数字表,将2012年6月—2015年1月收治的120例全髋关节置换术患者分成两组,每组60例。对照组患者按照术后常规护理要求进行护理,直至患者出院;观察组患者护理过程中贯穿连续性护理模式,并延续至家庭护理,比较出院后3个月患者日常生活能力、自我护理能力以及生存质量的影响。结果观察组出院3个月后ESCA评分和ADL评分分别为(122.8±13.7)分和(45.7±6.6)分,优于对照组,且明显优于护理前(P<0.05);出院后3个月,患者生理、心理、健康行为、环境问题评分分别为(89.2±5.3)分、(88.5±1.5)分、(88.3±6.2)分、(87.5±3.2)分,优于前两个阶段评分,各阶段评分均优于相应阶段对照组评分(P<0.05)。结论连续性护理模式对患者进行连续性跟进,可助于护理结局的整体提升,改善患者远期生存质量,推广价值较高。Objective To discuss the construction mode and application value of continuous nursing model in patients with total hip arthroplasty. Methods 120 cases of patients with total hip replacement treated from June 2012 to January 2015 were divided into two groups with 60 cases in each according to the random number table with their admission numbers re-garded as serial numbers, the control group were given nursing according to the conventional nursing requirements after op-eration until the patients were discharged from the hospital, continuous nursing model was throughout the nursing process of the observation group and extended to the family care, 3 months after discharge ,activity of daily living, self-care ability and survival quality of the patients were compared. Results 3 months after discharge, ESCA score and ADL score in the obser-vation group were respectively （122.8±13.7） and （45.7±6.6）, which were better than the control group and significantly better than those before nursing （P〈0.05）, 3 months after leaving hospital , physiological, psychological, health behavior, environ-mental problems scores of the patients were respectively （89.2±5.3）, （88.5±1.5）, （88.3±6.2） and （87.5±3.2）, which were better than the former two phases of scores, scores of each stage in the observation group were better than those in the control group（P〈0.05）. Conclusion Continuous nursing model can carry out the continuing follow-up to the patient, which can con-tribute to the overall improvement of nursing outcomes and improving the long-term survival quality of the patients with its high promotion value.
