摘　　要：目的:探讨个案管理在我国的应用情况及以后发展的前景。方法:翻阅有关文献,并结合自己的临床经验进行综述。结果:个案管理在慢性心力衰竭、腹膜透析和社区等方面都有研究,它的作用都是得到肯定的。结论:个案管理可以在我国应用,但需要探索一个适合我国的个案管理模式。Objective： To explore the application of case management in our country and the prospects for development. Methods： Through literatures, combined with his own clinical experience is summarized. Results： Its function that Case management in chronic heart failure, peritoneal dialysis and community studies is sure. Conclusions： Case management can be applied in our country, but it need to explore a suitable for the case management mode of our country.
