连续性血液净化治疗糖尿病乳酸性酸中毒临床观察

摘　　要：目的观察连续性血液净化治疗糖尿病乳酸性酸中毒临床效果。方法选取2010年1月~2014年12月我院治疗的32例糖尿病乳酸性酸中毒患者,随机分为观察组和对照组,各16例,对照组用常规药物治疗,观察组用连续血液净化治疗,观察及比较两组血生化指标、乳酸、二氧化碳结合力、p H值、住院时间及预后情况。结果两组患者治疗后血p H值、二氧化碳结合力及乳酸较治疗前均有所好转,且观察组治疗后乳酸水平下降更明显（P〈0.05）;两组治疗后血肌酐,尿素氮和钾水平均有所下降,但观察组下降更明显（P〈0.05）;观察组住院时间及死亡率均低于对照组（P〈0.05）。结论对糖尿病乳酸性酸中毒患者采用连续血液净化治疗,能有效清除患者体内蓄积的乳酸,促进患者恢复。Objective To observe the clinical effect of continuous blood purification in the treatment of diabetic lactic acidosis. Methods 32 patients of diabetic lactic acidosis in our hospital from January 2010 to December 2014 were randomly divided into observation group and control group,each group with 16 cases,the control group were given routine drug treatment,the observation group was given continuous blood purification therapy.The blood biochemical parameters,lactic acid,carbon dioxide combining power,p H value,length of hospital stay and prognosis of the two groups were observed and compared. Results After treatment,the carbon dioxide combining power,p H value and lactic acid levels of patients in the two groups improved significantly compared with before treatment,and the lactic acid level of the observation group after treatment decreased significantly compared with the control group（P〈0.05）;the serum creatinine,urea nitrogen and potassium levels of patients in the two groups after treatment decreased,but the observation group decreased more significantly（P〈0.05）;the duration of hospitalization and mortality rate in the observation group were lower than the control group（P〈0.05）. Conclusion Continuous blood purification in the treatment of patients with diabetic lactic acidosis,can effectively remove the body of lactic acid,promote the recovery of patients.
