64-MSCT及TTE对肺静脉异位引流的诊断分析

摘　　要：目的：探讨64层螺旋CT（64-slice computed tomography,64-MSCT）及经胸超声心动图（transthoracic echocardiography,TTE）对各类肺静脉异位引流诊断的效果评价。方法：对59例经手术证实的肺静脉异位引流患者的64-MSCT及TTE影像资料进行分析。结果：64-MSCT检查确诊57例,诊断准确率96.6%,TTE诊断正确51例,准确率为86.4%,2种检查的诊断准确性有显著性差异（χ~2=3.933,P=0.047）。Kappa检验显示二者一致性强度较弱（P=0.000,Kappa=0.366）。术中所见合并其他畸形共159处,64-MSCT诊断灵敏度为95.6%,TTE灵敏度为79.25%,2种检查的诊断准确性有显著性差异（χ~2=19.332,P〈0.001）。结论：64-MSCT可作为肺静脉异位引流术前的重要检查方法之一,为临床手术治疗提供重要依据。Objective To explore the values of 64-slice spiral CT（64-MSCT） and transthoracic echocardiography（TTE） for the diagnosis of types of anomalous pulmonary venous connections.Methods Totally 59 anomalous pulmonary venous connection patients confirmed surgically had their 64-MSCT and TTE imaging data analyzed.Results 64-MSCT found 57 cases of anomalous pulmonary venous connection,with the accuracy as 96.6%,while TTE found 51 cases of anomalous pulmonary venous connection,with the accuracy as 86.4%,and there was statistical difference between the accuracies of 64-MSCT and TTE（ χ~2=3.933,P =0.047）.Kappa test showed that there was weak consistency between 64-MSCT and TTE when used to diagnosing anomalous pulmonary venous connections（P=0.000,Kappa=0.366）.There were 159 other malformations discovered during the drainage operation,64-MSCT had the sensitivity as 95.6% and TTE had the sensitivity as 79.25%,with significant difference between the sensitivities（ χ~2=19.332,P0.001）.Conclusion 64-MSCT can be used for preoperative examination of the drainage of anomalous pulmonary venous connection to provide references for clinical operation.
