关节镜微创技术在儿童早期急性化脓性髋关节炎中的临床应用

摘　　要：目的探讨关节镜微创技术在儿童早期急性化脓性髋关节炎中的临床应用及体会。方法对关节镜诊治儿童早期急性化脓性髋关节炎8例病例,术后评估患者症状改善及并发症发生情况。结果所有病例均获随访,随访时间3~12个月。全部患者术后1周内症状消失,血常规及CRP、ESR等感染指标基本恢复正常。术后Harris髋关节评分（89±3.16）分,VAS评分（1.4±0.8）分,与术前Harris评分（52.10±6.50）分、VAS评分（7.8±1.1）分相比显著提高（P〈0.05）。全部患者术后无血管、神经损伤,无髋关节功能障碍、病理性髋脱位、髋关节迁延性感染及股骨近段骨髓炎、股骨头骨骺坏死等并发症发生。结论关节镜下微创手术治疗儿童早期急性化脓性髋关节炎疾病,具有诊断准确、创伤小、操作安全可靠、疗效好的优点,有利于患者早期功能康复。Objective To explore the clinical application of arthroscopy minimally invasive technique in treatment with early septic arthritis of the hip in children. Methods Eight cases with early septic arthritis of the hip in children diseases who received arthroscopic surgery were collected. The postoperative improvement and complications were evaluated. Results All patients were followed up from 3 to 12 months. All patients＇ symptoms disappeared within one week postoperatively. The CBC,CRP and ESR of the patients returned to normal postoperatively. The postoperative Harris hip score was 89 ± 3. 16 compared with the preoperative score 52. 10 ± 6. 50,and the VAS score was 1. 4 ± 0. 8 compared with the preoperative score 7. 8 ± 1. 1（ P 〈0. 05）. There were no complications of vascular,nerve injuries,dysfunction,pathologic dislocation,delayed infection,proximal femoral osteomyelitis and femoral head epiphysis necrosis cases happened during the follow- up. Conclusion For the treatment with early septic arthritis of the hip in children,hip arthroscopy has the advantages in safety,accurate diagnosis and minimally invasive with well efficacy,which is benefit for patients with early functional recovery.
