手足口病重症患儿感染71型肠道病毒后脑脊液的表现研究

摘　　要：目的 分析手足口病重症患儿感染71型肠道病毒（EV71）后脑脊液的临床表现。方法 将45例感染EV71的手足口病重症患儿作为EV71感染阳性组,将同期入院的其它肠道感染的21例手足口病重症患儿（EV71阴性）作为对照组。患儿入院1~5天行脑脊液常规和生化等检查,并观察其临床症状表现。结果 EV71感染组患儿发热、皮疹等临床症状发生率均显著高于对照组（P〈0.05）。EV71感染组白细胞数、单核细胞数均显著高于对照组（P〈0.05）,两组多核细胞数比较差异不显著（P〉0.05）。EV71感染组脑脊液中葡萄糖、蛋白质水平均显著高于对照组（P〈0.05）,氯化物水平略低于对照组,但差异不显著（P〈0.05）。结论 感染71型肠道病毒的手足口病重症患者临床症状明显,且脑脊液监测的异常值要高于其他肠道感染的手足病患儿。Objective To analyze the clinical manifestations of cerebrospinal fluid（CSF） of children with severe hand, foot and mouth disease（HFMD） infected with enterovirus 71（EV71）. Methods 45 severe HFMD children infected with enterovirus 71 were set as EV71 infection group（positive EV71）, and 21 severe HFMD children with other intestinal infection（negative EV71） were set as control group. The routine and biochemical test on CSF were performed 1 ~ 5 d after admitting to hospital, and the clinical manifestations were observed. Results The incidence of fever and rash of EV71 infection group were significantly higher than those of control group（P〈0.05）.The count of leukocytes and monocytes of EV71 infection group were significantly higher than those of control group（P〈0.05）, but the count of coenocytes of two groups had no significant difference（P〉0.05）. The levels of glucose and protein of EV71 infection group were significantly higher than those of control group（P〈0.05）, and the level of chloride was lower than that of control group, but the difference was not statistical（P〈0.05）. Conclusions The clinical manifestations of severe HFMD children infected with EV71 are significant, and the abnormal value of CSF is higher than severe HFMD children with other intestinal infection.
