外用象皮生肌膏治疗不可分期压疮的效果观察

摘　　要：目的:观察外用象皮生肌膏治疗不可分期压疮的效果。方法:按入选标准将60例患者随机分为两组,每组30例。按操作流程评估和清创,治疗组全程给予象皮生肌膏至愈合,对照组根据创面不同分期选择合适的新型敷料:水凝胶、银离子、水胶体敷料等覆盖至愈合。从初诊至治疗结束,全程给予局部减压、每2 h翻身1次、营养支持、活动和皮肤护理等全身干预。分别于治疗第7、14、21、28天记录两组的压疮面积、创面分泌物和组织类型,进行压疮愈合PUSH计分,并对结果进行统计学分析。结果:治疗第7天,治疗组PUSH计分为(15.07±0.18)分、对照组为(14.23±0.13)分,两组比较,差异有统计学意义(P<0.01);治疗第14天,治疗组PUSH计分为(14.03±0.21)分,对照组为(14.70±0.13)分,两组比较,差异有统计学意义(P<0.01);治疗第21、28天,治疗组PUSH计分分别为(11.50±0.21)分、(8.53±0.13)分,对照组分别为(11.50±0.22)分、(8.60±0.11)分,两组比较,差异均无统计学意义(P>0.05)。结论:外用中药象皮生肌膏治疗不可分期压疮有较好的祛腐溶痂、促进压疮创面修复的效果。Objective： To observe the efficacy of Xiangpishengjigao on non stage pressure sore. Methods： According to the criteria, the 60 patients were randomly divided into 2 groups, 30 in each. According to the operation process evaluation and debridement treatment, patients were treated by external use of Xiangpishengjigao for healing in treatment group; and patients in control group were given appropriate new wound dressings with hydrogels, silver ions and hydrocolloid dressings covering according to different stages. From the first diagnosis until the end of treatment, patients were given all the local decompression, falt nursing （1 time every 2 h）, nutritional support, activities, and intervention all over the body such as the skin care. The pressure ulcer area, wound secretion and tissue types for pressure ulcer healing PUSH score were record in two groups, respectively in the treatment of 7 d, 14 d, 21 d and 28 d, and the results were statistically analyzed. Results： The PUSH the scoring was （15.07±0.18） scores in treatment group and （14.23±0.13） scores in control group at the 7th day, and the comparison indicated statistical difference （P〈0.01）; The PUSH the scoring was （14.03±0.21） scores in treatment group and （14.70±0.13） in control group at the 14th day, and the comparison indicated statistical difference （P〈0.01）; The PUSH the scoring was （11.50±0.21） and （8.53±0.13） scores in treatment group respectively at 21st and 28th day, and （11.50±0.21） and （8.60±0.11） scores in control group; the comparison indicated no statistical difference （P〉0.05）. Conclusion： External use of Xiangpishengjigao can provide definite clinical curative effect on non stage pressure sores, remove corrosion dissolved scab, and promote the ulcer wound repair.
