参附黄蒲汤治疗脾胃虚寒型胃痛随机对照临床研究的Meta分析

摘　　要：目的:评价参附黄蒲汤治疗脾胃虚寒型胃痛的临床疗效。方法:检索EMBASE、PUBMED、MEDLINE、AMED、CINAHL、Cochrane图书馆、中国知网数据库、重庆维普中文科技期刊数据库、中国生物医学文献数据库、万方数据库和中国中医药期刊文献数据库从建库至2015年1月30日参附黄蒲汤治疗脾胃虚寒型胃痛随机对照试验研究,筛选合格研究。应用Cochrane评价员手册进行偏倚风险评价,Jadad评分法进行质量评价。应用Rev Man5.3软件进行Meta分析。结果:符合纳入标准的文献共7篇。Meta分析结果显示,参附黄蒲汤治疗脾胃虚寒型胃痛治疗组与对照组相比,总有效率比值比(OR)=7.90,95%可信区间(CI)[3.98,15.71],差异有统计学意义(P<0.00001);治愈率OR=1.51,95%CI(1.12,2.05),差异有统计学意义(P=0.008);症状评分均数差(MD)=-2.36,95%CI(-3.26,-1.46),差异有统计学意义(P<0.00001);纳入研究未报道不良反应。结论:参附黄蒲汤治疗脾胃虚寒型胃痛临床疗效显著,但由于本系统评价纳入研究的总体质量较低,研究的数量和样本量均较小,尚需要更多高质量、多中心、大样本、严格设计的随机对试验来进一步验证其疗效与安全性。Objective： To evaluate the clinical efficacy of Shenfu Huangpu decoction in the treatment of stomachache of spleen deficiency syndrome. Methods： EMBASE, PUBMED, MEDLINE, AMED, CINAHL, Cochrane Library, China National Knowledge Infrastructure Database, Chongqing VIP Chinese Science and Tech- nology Periodical Database, Chinese Biomedical Literature Database, Wanfang Database and Traditional Chinese Medical Periodical Literature Database from the inception of each database up to 30 January 2015 were elec- tronically searched. Randomized clinical trials （RCTs） of Shenfu Huangpu decoction for stomachache of spleen deficiency syndrome were included. The Cochrane risk of bias tool was used to assess the risk of bias. We as- sessed the quality of included studies using the Jadad rating scale. The Cochrane Collaboration＇s RevMan5.3 software was used for meta-analysis. Results： There were 7 studies included for meta-analysis. The meta-analy- sis results showed that Shenfu Huangpu decoction experiment group comparing with control group, the odds ra- tio（OR） of clinical total efficiency rate was 7.90, with 95% confidence interval （C/） [3.98,15.71], and the differ- ence was statistically significant （P〈0.00001）; the cure rate OR was 1.51, with 95% Cl[1.12,2.05],and the dif- ference was statistically significant （P=0.008）; symptom score mean difference （MD） was -2.36, with 95% CI [-3. 26, -1.46], and the difference was statistically significant （P〈0.00001）. All studies included did not report ad- verse reactions. Conclusion： Shenfu Huangpu decoction showed significant clinical curative effect on stomachache of spleen deficiency syndrome, However, the number and the study sample size are small and the methodological quality is low. More rationally designed and strictly executed RCTs with large samples need to be made, in order to further evaluate the effect and safety of Shenfu Huangpu decoction for stomachache of spleen deficiency syndrome.
