彩色多普勒超声诊断下肢深静脉血栓与肺栓塞的相关性

摘　　要：目的探讨彩色多普勒超声诊断下肢深静脉血栓与肺栓塞发生率的相关性,为临床预防和治疗提供依据。方法选择解放军总医院第一附属医院2013年5月至2014年5月收治的肺栓塞患者40例(PE组),采用彩色多普勒超声评价肺拴塞患者下肢深静脉血栓形成的发生率。另选取同期在我院接受治疗的下肢深静脉血栓患者40例(DVT组),采用超声诊断下肢深静脉血栓患者合并肺栓塞的情况。结果 PE组40例患者共检出下肢深静脉血栓33例,肺栓塞合并下肢深静脉血栓的超声检出率为82.50%。左下肢形成深静脉血栓的比例高于右下肢,差异有统计学意义(P<0.05)。DVT组40例患者共检出肺栓塞32例,下肢深静脉血栓合并肺栓塞的超声检出率为80%。双侧肺栓塞的比例高于单侧肺栓塞,差异有统计学意义(P<0.05);急性肺栓塞的比例高于亚急性和慢性肺栓塞,差异均有统计学意义(P<0.05)。肺栓塞直接征象以充盈缺损最多,间接征象以肺动脉扩张和胸腔积液出现率最高。结论彩色多普勒超声能够准确地显示静脉结构、血栓部位、阻塞程度、血管周围组织及血流动力学信息,对下肢深静脉血栓形成和肺栓塞有很高的诊断价值。Objective To quantitatively analyze the ultrasound elastography system for the diagnostic value of deep venous thrombosis （DVT） and pulmonary embolism （PE）,and to evaluate the correlation between the two diseases,for providing the basis for clinical prevention and treatment. Methods Forty patients with PE （ PE group） were treated in our hospital from May 2013 to May 2014 were treated 40 cases,and color Doppler ultrasound was used to evaluate the incidence of PE in patients with DVT. Also during the same pe-riod in our hospital,40 cases （ DVT group） were enrolled in this study for the treatment of DVT,and ultrasonic determination was per-formed on DVT patients complicated with PE. Results DVT had been detected in 33 cases of the PE group,and the detection rate was 82. 5% （33/40）. Occurrence rate of DVT in left lower extremity was higher than that in right side （P〈0. 05）. PE had been detected in 32 case of the DVT group,and the detection rate was 80. 0% （32/40）. Occurrence rate of bilateral PE was higher than lateral cases （P〈0. 05）,and occurrence rate of acute PE was higher than sub-acute and chronic cases （P〈0. 05）. For PE manifestation,filling defects were the most direct sign,while,dilated pulmonary artery and pleural effusion were the most indirect sign. Conclusion Ultra-sound elastography can accurately display the vein structure,thrombus,blocking extent,perivascular tissue and hemodynamic informa-tion. It is valuable for the diagnosis of DVT and PE.
