华东地区正常成人标化上气道CT影像测量值的建立

摘　　要：目的采用标准化CT检查法建立华东地区成人上气道各区域CT影像的正常参考值。方法招募300名华东地区正常成年人志愿者,行标化上气道CT平扫及多平面重建,分别测量鼻咽、腭咽、舌咽及下咽四个区域的横截面积、矢状径、冠状径及腭咽区和舌咽区最狭窄平面的CT影像横正常参考值(单侧95%可信区间)。结果青年男性上气道所测各区域正常参考值下限分别为鼻咽区251.1 mm^2,腭咽区118.3 mm^2,舌咽区141.5 mm^2,下咽区137.9 mm^2,腭咽最狭窄区61.4 mm^2,舌咽最狭窄区117.1 mm^2;中年男性腭咽区下限为125.1 mm^2,舌咽区149.7 mm^2,舌咽最狭窄区125.0 mm^2,其余区域与青年男性相同。青中年非绝经期女性该类数值分别为225.6 mm^2、96.9 mm^2、112.8 mm^2、108.8 mm^2、56.6 mm^2、100.9 mm^2;中年绝经期女性鼻咽区下限为245.1 mm^2,其余区域与非绝经期女性相同。结论华东地区正常成年人上气道大小存在性别及年龄差异,女性行经状态等因素对相关数值有显著影响。该类参考值有助于该地区OSAHS人群上气道狭窄部位的判定及手术方案的制定。Objective To determine the normalized normal upper airway （UA） image reference value ranges by CT scan among healthy adults in eastern China. Methods Included in this study were 300 healthy adult volunteers recruited from eastern China, with their UA image reconstructed at multi-planes following CT scanning assisted by head-holding device. Then, image data of UA were collected to determine the normalized normal image reference value ranges. Here, UA was divided into four regions, i.e. nasopharyneal （NP）, palatopharyngeal （PP）, glossopharyngeal （GP）, and hypopharyngeal （HP） regions on the images to calculate their areas at cross- sectional plane （CSP）, sagittal diameters （SD）, coronal diameters （CD）, and the narrowest diameters of PP region and GP regions to set up their normal image reference value ranges, with one side 95% confidence interval statistically calculated. Results The lower limits of these cross-sectional areas determined in UA of young men were 251.1 mm^2 in NP region, 118.3 mm^2 in PP region, 141.5 mm^2 in GP region, and 137.9 mm^2 in HP region respectively, with the narrowest areas being 61.4 mm^2 in PP region and 117.1 mm^2 in GP region, while the lower limits of cross-sectional areas among middle-aged men were 125.1 mm^2 in PP region, 149.7 mm^2 in GP region, and the narrowest area was 125.0 mm^2 at the plane of GP region, with other area values being the same as that of young men. Among young and middle-aged non menopausal women, the lower limits of these values were 225.6 mm^2, 96.9 mm^2, 112.8 mm^2,108.8 mm^2, 56.6 mm^2 and 100.9 mm^2 respectively for those locations as that of young men in the same order, while the area was 245.1 mm^2 in NP region among middle-aged menopausal women, with other values being the same as that in Young and middle-aged non menopausal women.
