摘  要:目的研究并探讨弥散张量成像(DTI)在足月新生儿窒息后脑损伤中的应用效果。方法选取2012年1月—2015年1月15例新生儿缺氧缺血性脑病(HIE)足月患儿作为研究对象,将其设置为A组;选取同时间段发生窒息但未出现脑损伤的15例足月患儿,将其设置为B组;另选取同期出生的15例足月正常儿,将其设置为对照组。使用磁共振扫描仪对这45例新生儿进行颅部MRI常规序列扫描和DTI序列扫描,对比3组新生儿颅部各感兴趣区的FA值。结果各组新生儿的内囊后肢的FA值差异有统计学意义(P<0.05),A组、B组的额叶白质、胼胝体压部、大脑脚、半卵圆中心的FA值均与对照组比较差异有统计学意义(P<0.05)。结论在足月新生儿窒息后脑损伤中应用DTI扫描,能够有效鉴别新生儿的窒息和脑损伤情况,其中内囊后肢区域的诊断价值最高。Objective To investigate the effect of diffusion tensor imaging (DTI) in the brain injury after neonatal asphyxia. Methods 15 cases of neonatal hypoxic ischemic encephalopathy (HIE) were selected as the research object in January 2012~January 2015, and the A group was set;15 cases of cerebral injury occurred in the same time, but not in the case of brain injury, and were set to B group;another 15 cases of normal children born in the same period were set up as the control group.Using magnetic resonance scanner, the routine sequence and DTI sequence of cranial MRI scan were performed in 45 cases, and compared with the FA value of the cranial part of the three groups.Results The neonatal FA value of the posteri-or limb of the internal capsule were significantly different (P〈0.05),the FA values of frontal lobe white matter, corpus callo-sum, brain, and semi oval center of A group and B group were significantly different from that of the control group (P〈0.05). Conclusion In full-term neonates with asphyxia brain damage in application of DTI scanning and can effectively identify neonatal asphyxia and brain injury, which the posterior limb of the internal capsule area highest diagnostic value.
