冠状动脉钙化积分对不稳定心绞痛患者行冠状动脉支架植入术后肌钙蛋白Ⅰ和脑钠肽升高的预测作用

摘　　要：目的:评价冠状动脉钙化积分(CACS)对不稳定性心绞痛患者左前降支支架植入后肌钙蛋白I(c Tn I)和B型脑钠肽(BNP)升高的预测作用。方法:选取行左前降支支架植入的480例不稳定性心绞痛患者,均术前测BNP和c Tn I,术前行64层冠脉CTA并CACS扫描,根据左前降支钙化程度将病例分为2组:A组315例(CACS<100分)和B组165例(CACS≥100分)。480例均在左前降支行支架植入术,术后24 h测BNP和c Tn I。结果:2组患者术前血c Tn I和BNP含量差异均无统计学意义(P>0.05);2组患者术后血c Tn I和BNP含量均较术前明显升高(P<0.01),而B组术后血c Tn I和BNP含量亦显著高于A组(P<0.01)。结论:高冠脉钙化积分(CACS≥100)对冠脉支架植入术后肌钙蛋白I和BNP升高有一定的预测价值。Objective ：To evaluate the forecasting function of the coronary artery calcium score （CACS） on the increasing of cardiac Troponin I（eTnI） and Brain Natriuretie Peptide（BNP） levels in unstable angina peetoris after left anterior descending artery stenting. Methods ： The levels of BNP and cTnI, and 64-slice coronary CTA scanning and CACS in 480 patients with unstable angina pectoris were detected before left anterior descending artery stenting. The patients were divided into group A （315 cases, CACS less than 100 points） and group B（ 165 cases,CACS equal to or more than 100 polnts） according to the left anterior descending artery calcification score. The levels of BNP and cTnI in all cases were measured after 24 h of left anterior descending artery stenting. Results：The differences of the preoperative levels of cTnI and BNP in group A and group B were not statistically significant （ P 〉 0.05 ）. The postoperative levels of cTnI and BNP in group A and group B were significantly higher than the preoperative levels（ P 〈 0.01 ） , and the postoperative levels of cTnI and BNP in group B were higher than those in group A （ P 〈 0.01 ）. Conclusions ： The high CACS has certain predictive value in the levels of cTnI and BNP after left anterior descending artery stenting.
