摘 要:目的探讨糖耐量受损(IGT)与正常糖耐量(NGT)者24h血糖漂移变化的特征。方法对5l例IGT及41例NGT进受试者行动态血糖监测,评估24h血糖漂移变化的差异。结果动态血糖谱显示,IGT组血糖均值[(6.48±0.11)mmol/L对(5.31±0.08)mmol/L]、血糖峰值[(9.26±0.44)mmol/L对(6.13±0.31)mmol/L]、极差[(7.10±0.37)mmol/L对(3.94±0.23)mmol/L]及MAGE[(3.52±0.26)mmoI/L对(2.04±0.12)mm01/L]均显著高于NGT组(P〈0.01)。IGT组日内血糖〉7.8mmol/L及8.9mmol/L的时间百分比值较NGT组分别增高7-27倍及16-37倍。结论动态血糖监测可详细地呈现IGT患者全天血糖漂移变化的细节及趋势,为早期合理的个体化干预IGT措施提供临床依据。Objective To investigate the characteristics of 24h glucose excursion in healthy subjects with normal glucose tolerance (NGT) and subjects with impaired glucose tolerance (IGT).Methods The difference of 24h glucose excursions was evaluated in 51 IGT and 41 NGT subjects respectively by continuous glucose monitoring (CGM) system.Results 1) In IGT group, average glycemic values [ (6.48±0.11 )mmol/L vs (5.31±0.08)mmol/L] ,daily glycemic peaks [ (9.26±0.44)mmol/L vs (6.13±0.31 )mmol/L], the differences between maximal and minimal glycemic values [(7.10±0.37)mmol/L vs (3.94±0.23)mmol/L] and mean amplitude of glycemic excursions (MAGE)[(3.52±0.26)mmol/Lvs(2.04±0.12)mmol/L] were all significantly higher than those in NGT group (P 〈 0.01). 2)The percentage time spent in hyperglycemia above 7.8mmol/L and 8.9mmol/L had raised 7.27 times and 16.37 times respectively.Conclusion Subjects with IGT showed obvious glucose metabolic disturbance compared with IGT group. Continuous glucose monitoring may reveal the characteristics of glucose excursion for IGT status in detail,which is beneficial to taking individual intervention management earlier.
