威伐光治疗术后慢性疼痛24例疗效观察

摘　　要：目的观察威伐光联合加巴喷丁和曲马多治疗慢性术后疼痛（CPSP）的临床疗效。方法将48例CPSP患者随机分为A．B两组，各24例。A组口服加巴喷丁和曲马多治疗，B组在此基础上，辅以威伐光照射治疗，观察并记录两组患者于治疗前（T0）、治疗1周（T1）、治疗2周（T2）、治疗4周（T3）4个时点临床效果及不良反应。结果两组患者在治疗后VAS评分均有所下降（P〈0．05）：B组在T1，T2，T3时点的VAS评分下降程度均低于A组（P〈0．05）。两组疼痛缓解有效率在T3时点无差异（P〉0．05）：B组显效率优于A组（P〈0．05）。两组睡眠质量评分在治疗后均有所增加（P〈0．05）；A组评分在，T2，T3时点较前一时点无增加趋势．B组则随治疗时间延长呈增长趋势。两组未见明显不良反应。结论威伐光联合加巴喷丁和曲马多治疗CPSP安全有效。Objective To observe the clinical efficacy of water-filtered infrared A （wIRA） irradiation in the treatment of Chronic post-surgical painsyndrome （CPSP）.nethods According to complete random design,48 patients with CPSP were divided into two groups：A （n=24）group ,B （n=24） group.the two groups were treated with gabapentin and tramadol and the B group was treated additionally with wlRA irradiation simultaneously. The efficacy and adverse reactions of pretreatment （TO） and 1 week（T1）, 2 weeks （T2）,4 weeks （T3） after treatment between the two groups were observed an recorded.Results The visual analogue scores （VAS） of two goups after the treatm had declined （P〈0.05）;Compared with the A group,VAS of B group were declined more significantly at T1 ,T2,T3 points.Two groups had no statistically significant difference on the effective rate of pain relieve,but- significant efficiency rate of B group was higher than A group （P〈0.05）.The quality of sleep cores of two groups had been in- creased （P〈0.05）;Compared with previous point,scores had no increased at T2 and T3 points,while B group had increased with the extension of treatment（P〈 0.05）.The adverse reactions of two groups had no significant difference（P〉 0.05）.Conclusion wIRA irradiation combined with gabapentin and tramado is safe and effective in the treatment of CPSP.
