多层螺旋CT导向经胸穿刺活检在肺部小结节诊治中的临床价值

摘　　要：目的观察和探讨64排螺旋CT导向肺部小结节经胸穿刺活检的准确性及临床应用价值。方法整群选取2013年12月—2015年6月该院收治的肺部小结节CT导向经胸穿刺活检者92例,其中经64排螺旋CT导向46例(观察组),经普通CT导向46例(对照组)。比较观察组和对照组肺部小结节穿刺活检在诊断正确率和并发症发生率的差异。结果观察组肺深部小结节穿刺活检诊断正确率91.30%(42/46)高于对照组的诊断正确率78.26%(36/46)差异有统计学意义(P<0.05)。观察组肺部小结节穿刺活检的并发症发生率10.87%(5/46)低于对照组的并发症发生率21.74%(10/46),两组差异有统计学意义(P<0.05)。结论与普通CT比较,64排螺旋CT导向肺部小结节穿刺活检是一种更安全、准确的诊断和鉴别诊断方法,值得临床推广应用。Objective To observe and discuss the accuracy and clinical application value of 64-multidetector spiral CT- guided percutaneous transthoracic biopsies in the diagnosis and treatment of lung nodules. Methods 92 cases of patients with lung nodules receiving CT-guided percutaneous transthoracic biopsies from December 2013 to June 2015 were selected and divided into two groups with 46 cases in each, patients receiving 64-muhidetector spiral CT-guided were regarded as the observation group, patients receiving common CT-guided were regarded as the control group, the differences in the diagnostic accuracy rate and incidence of complication of aspiration biopsy of lung nodules between the two groups were compared. Results The diagnostic accuracy rate in the observation group was obviously higher than that in the control group （91.30%（42/46）vs 78.26%（36/46））and the difference was statistically significant （P〈0.05）,the incidence of complication in the observation group was lower than that in the control group,（10.87% （5/46）vs 21.74% （10/46））, and the difference be- tween the two groups was statistically significant （P〈0.05）. Conclusion Compared with common CT, 64-multidetector spiral CT-gnided percutaneous transthoracic biopsies is a more safe and accurate diagnostic and differential diagnosis method, which is worthy of promotion and application.
