肠外营养支持用于脑梗死患者29例及护理体会

摘  要:目的探讨脑梗死患者肠外营养应用及护理方法。方法选择2008年12月至2012年12月神经内科住院并接受肠外营养支持的老年脑梗死患者29例,均经锁骨下静脉置入中心静脉导管,对血清总蛋白、血清白蛋白、血清前蛋白、血红蛋白及体重指数(BMI),患者拔管数及导管血培养情况进行统计分析。结果29例患者营养状况均有不同程度的改善,治疗前后总蛋白和BMI均有统计学差异(P〈0.05),其余指标均无统计学差异。因不明原因高热拔管2人次,导管尖端及血培养情况均为阴性。结论经过正规化的肠外营养支持治疗,脑梗死患者的营养状况均有改善,而严格有效的导管护理可有效减少肠外营养患者导管感染的发生率。Objective To explore the Parenteral nutrition (PN)and Nursing in patients with cerebral infarction.Methods A total of 29 patients who were admitted to the Department of Neurology,PLA General Hospital,and received PN from December 2008 to December 2012 were enrolled in this studySubclavian vein cannulation was achieved in all of these patients and the total serum protein,serum albumin,serum precursor protein,hemoglobin and BMI index,the number of patients with extubation and catheter blood cultures were analysised.Results The nutritional status of 29 patients were improved.There were significant differences of total protein and BMI index in the two groups (P〈0.05).Total 2 of 29 patients were removed the catheters due to unexplained fever,but a negative blood culture and catheter tip results were obtained.Conclusion After PN,the nutritional status of patients with cerebral infarction were improved.Strict catheter maintenance can effectively reduce PN-related infections.
