手术联合放化疗综合治疗软组织肿瘤的疗效分析

摘　　要：目的探讨软组织肿瘤手术治疗联合化疗或放疗综合治疗的疗效分析。方法选择2008年至2011年收治的43例软组织肿瘤患者，分析X线、病理学和组织学检查结果以及治疗方法。11例患者采用单纯手术治疗。进行肿瘤的广泛切除：14例患者采用术前放疗＋手术切除＋术后放疗；9例采用术前顺铂化疗＋手术切除＋术后顺铂化疗：9例患者术前放疗＋手术切除＋术后顺铂化疗＋术后放疗。结果7例患者肿瘤复发，其中1例足底纤维瘤患者，经过3次复发并广泛切除后截肢治疗。所有患者3年生存率为69．77％．脂肪瘤患者生存率较好，为100．00％。结论术前进行X线、病理学和组织学检查，对于软组织肿瘤治疗方法的选择十分重要。依据患者具体病情选择合适的治疗方法能提高临床疗效，其中术前放疗＋手术切除＋术后顺铂化疗＋术后放疗治疗恶性肿瘤效果较好。Objective To investigate the efficacy of surgical treatment of soft tissue tumors in combination with chemotherapy and/ or radiotherapy therapy.Methods 43 cases with soft tissue tumors from 2008 to 2011 were received in our hospital.Basing on X- ray,pathology and histology results,we used different treatment methods.Eleven patients were treated with surgery alone with wide resection;Fourteen patients were treated with preoperative radiotherapy＋surgery＋postoperative radiotherapy;Nine cases were treated with preoperative cisplatin＋surgery＋postoperative cisplatin;Nine patients were treated with preoperative radiotherapy＋surgery＋postop- erative radiotherapy＋postoperative eisplatin.Results 7 patients got tumor recurrence ,including one case of plantar fibroma patient. After three times relapse and resection,we took amputation resection.3-year survival rate for all patients was 69.77%,patients with lipoma better survival rate of 100.00%.Conclusion Pre-surgery of X-ray,pathology and histology had significant effects on the choice of treatment for soft tissue tumors.Basing on patient-specific conditions,selecting appropriate treatment can improve clinical efficiency.However,preoperative radiotherapy＋surgery＋postoperative radiotherapy＋postoperative eisplatin can have better clinical effi- ciency for malignant soft tissue tumors.
