注射用益气复脉(冻干)联合西药常规治疗慢性心力衰竭疗效的Meta分析

摘　　要：目的系统评价注射用益气复脉(冻干)联合西药常规治疗慢性心力衰竭的临床疗效与安全性。方法计算机检索中国期刊全文数据库、中文科技期刊全文数据库、万方数据库、中国生物医学文献数据库、Embase、Pubmed、The Cochrane Library,检索时间均从数据库建库至2015年1月。纳入注射用益气复脉(冻干)联合西医常规与西医常规对照治疗心力衰竭的临床随机对照研究。使用统一资料提取表提取患者一般资料、干预措施、观察指标等信息,采用Cochrane协作网提供的偏倚风险评估工具对纳入研究进行方法学质量评价。采用Rev Man 5.2.6对患者临床综合疗效、心功能分级、左室射血分数、左室舒张末内径、氨基末端前脑钠尿肽进行Meta分析。结果共纳入18篇文献,1939例患者,文献质量偏低。Meta分析结果显示,加用注射用益气复脉(冻干)治疗慢性心力衰竭可以提高患者临床综合疗效[OR=3.63,95%CI(2.22,5.93),P<0.001],改善心功能[OR=1.98,95%CI(1.48,2.65),P<0.001],增加左室射血分数[WMD=4.35,95%CI(3.32,5.38),P<0.001],降低左室舒张末内径[WMD=-2.59,95%CI(-4.97,-0.21),P<0.03],降低氨基末端前脑钠尿肽[WMD=-298.41,95%CI(-453.86,-142.97),P=0.0002]。结论在纳入研究的范畴内,注射用益气复脉(冻干)联合西医常规治疗慢性心力衰竭可进一步改善患者症状和心功能,提高临床疗效。Objective To systematically evaluate clinical effects and safety of Yiqi Fumai Injection （lyophilization） combined with conventional western medicine for chronic heart failure. Methods China journal full-text database, Chinese technical journal full-text database, Wanfang database, Chinese biomedical literature database, EMBASE, PubMed and The Cochrane Library were searched from the time databases were built until January 2015 to identify the randomized controlled clinical studies on Yiqi Fumai Injection （lyophilization） combined with conventional western medicine to treat heart failure. The unified forms were used to collect information such as patients＇ general data, intervention measure and observational index. The bias-risk assessment tools provided by Cochrane collaboration network were applied to make methodological quality assessment for the including studies. RevMan 5.2. 6 was used for Meta analysis on clinical comprehensive effects, cardiac functional grading , left ventricular ejection fraction, left ventricular end-diastolic diameter and amino-terminal pro-brain natriuretic peptide of patients. Results Eighteen articles were indentified, with 1939 patients involved in. The quality of literature was poor. Results of meta analysis showed that combined with Yiqi Fumai Injection （lyophilization） into the treatment of chronic heart failure may improve clinical comprehensive effects [ OR = 3.63, 95% CI （2.22, 5.93 ）, P 〈 0. 001 ], improve cardiac function [ OR = 1.98, 95% CI （ 1.48, 2.65）, P 〈 0. 001 ], increase left ventricular ejection fraction [ WMD = 4. 35, 95% CI （3.32, 5.38）, P 〈 0. 001 ], lower left ventricular end-diastolic diameter [ WMD = -2.59, 95% CI （ -4. 97, -0. 21 ） , P 〈 0.03 ] , and reduce amino-terminal pro-brain natriuretic peptide [ WMD = - 298.41, 95% CI （ -453.86, - 142.97 ）, P0. 0002 ）.
