黄芪注射液对脑病治疗的meta分析

摘　　要：目的全面系统地评价黄芪注射液对脑病的治疗作用。方法检索万方数据库,CNKI,中国生物医学文献数据库(CBM),中文科技期刊全文数据库(VIP),以及Web of Knowledge对黄芪注射液治疗脑病的研究。采用Cochrane协作网专用软件Rev Man 5.3软件进行meta分析。结果纳入27篇文献,Jadad评分均小于2,文献质量较低,对黄芪注射液对脑梗塞、脑梗死的meta分析黄芪注射液组临床疗效高于对照组[RR=1.60,95%CI(1.31,1.94),P<0.000 01],[RR=1.58,95%CI(1.44,1.74),P<0.000 01];黄芪注射液与丹参制剂的联用对于脑病的治疗效果更为显著[RR=1.52,95%C(I1.36,1.71),P<0.000 01];对于脑病的总的治疗,治疗组与对照组存在显著差异[RR=1.56,95%CI(1.46,1.67),P<0.000 01];对其作用机制的研究发现黄芪注射液能有效提高神经功能的修复[MD=-4.71,95%CI(-6.24,-3.17),P<0.000 01];倒漏斗图存在偏倚的可能较小。结论黄芪注射液在脑病治疗中是安全有效的,为黄芪注射液在脑病治疗中更为广泛的运用提供了较为可靠的分析支持,但仍需高质量随机对照临床验证。Objective Evaluating the therapeutic effect of Huangqi injection on the encephalopathy systematically. Methods We searched Wanfang database, CNKI, Chinese biomedical literature database （ CBM ）, Chinese Scientific Journals Full-text Database （ VIP ）, and Web of Knowledge about trials of Huangqi Injection in the treatment of encephalopathy. A meta-analysis was performed using RevMan 5.3 software. Results A total of 13 studies involving 2 596 patients were included in this study. All articles were less than 2 and the overall study quality was lower according to the Jadad scale. Both meta analysis about cerebral embolism and cerebral infarction showed the group using Huangqi injection is higher than the control group[ RR = 1.60, 95% CI （ 1.31,1.94 ）, P 〈 0.000 01 ], [ RR = 1.58, 95% CI （ 1.44,1.74 ）, P 〈 0.000 01 ]; the combination of Huangqi injection and Danshen preparations group can enhance the curative effect of encephalopathy[ RR = 1.52, 95% CI （ 1.36, 1.71 ）, P 〈 0.000 01 ]; Meta analysis of encephalopathy treatment indicates the expeiremental groups and control groups exist significant differences[ RR = 1.56, 95% CI （ 1.46, 1.67 ）, P 〈 0.000 01 ]; The research on the mechanism of its mechanism found that Huangqi injection can improve the nerve function repair effectively[ MD = - 4.71, 95% CI （ - 6.24, - 3.17）, P 〈 0.000 01 ]; The funnel plot shows this article exists a certain bias. Conclusion Huangqi injection can promote the recovery of neural function. Using Huangqi injection in the treatment of encephalopathy is safe and effective, the paper provides a reliable analysis support to ensure the use of Huangqi injection more widely, but there still need some high quality randomized controlled trails to verificate the validity and correctness of the paper.
