盐酸羟考酮缓释片联合复方苦参注射液治疗癌性神经病理性疼痛

摘　　要：目的:观察盐酸羟考酮缓释片联合复方苦参注射液治疗癌性神经病理性疼痛患者的近期疗效和不良反应。方法:将72例癌性神经病理性疼痛患者随机分为两组各36例,治疗组口服盐酸羟考酮缓释片40 mg,q 12 h,联合复方苦参注射液30 ml稀释后静脉滴注,qd。对照组口服盐酸羟考酮缓释片40 mg,q 12 h。两组治疗后第3、7和14 d进行疗效比较。结果:治疗后第7 d治疗组、对照组疼痛治疗有效率分别为88.9%和61.1%(P=0.014);治疗后第14 d治疗组、对照组疼痛治疗有效率分别为91.7%和75.0%;差异均有统计学意义(P<0.05)。14 d内人均羟考酮剂量调整次数,治疗组(1.44±1.40)次,对照组为(2.47±1.97)次,差异有显著性(P<0.05)。治疗组恶心呕吐发生率为33.3%,显著低于对照组恶心呕吐发生率61.1%(P<0.05)。结论:盐酸羟考酮缓释片联合复方苦参注射液治疗癌性神经病理性疼痛效果良好,并可降低恶心呕吐发生率。Objective： To investigate the curative effect and safety of compound matrine injection combined with oxycodone hydrochloride sustained- release tablet in the treatment of malignant neuropathic pain. Methods： Patients with malignant neuropathic pain were divided randomly into the control group in which the patients were given oxycodone hydrochloride only（ 40 mg,q12 h） and into the treatment group in which both compound matrine injection（ 30 ml,ivgtt,qd） and oxycodone hydrochloride tablet were administered to the patients. The analgesic effect was assessed on Days 3,7 and 14 during the treatment course,respectively. Results： The analgesic effect in the treatment group was greater than that in the control group,the effective rate of the treatment group on Day 7 being significantly higher than that of the control group（ 88. 9% vs 61. 1%） and on Day 14（ 91. 7% vs 75. 0%）. Within 14 days,the per capita times of dose adjustments of oxycodone hydrochloride in the treatment group was significantly fewer than those in the control group（ 1. 44 ± 1. 40 vs 2. 47 ± 1. 97）. The incidence of nausea and vomiting in the treatment group was significantly lower than that in the control group（ 33. 3% vs 61. 1%）. Conclusion： Compound Matrine Injection is effective in assisting oxycodone hydrochloride in the treatmnent of malignant neuropathic pain,and has less nausea and vomiting effects.
