摘　　要：目的：对脐血Ig E升高的过敏高危婴儿及普通婴儿进行研究,探讨喂养干预对婴儿湿疹（infantile eczema IE）发病的影响。方法：用酶联免疫（ELISA）法检测新生儿脐血Ig E,将脐血Ig E≧1 IU/ml的60例归入高危组,Ig E〈1 IU/ml的60例归入普通组,2组再各分为干预组和对照组各30例,干预组进行喂养干预,主要包括母乳喂养至少6个月,4~6个月时添加辅食等;对照组母乳或人工喂养,6个月后添加辅食。4组婴儿均追踪12个月,记录IE的累积发病率。结果：12个月内IE的累积发病率：高危干预组30.0%（9/30）,高危对照组73.3%（22/30）,普通干预组13.3%（4/30）,普通对照组33.3%（10/30）,即高危对照组发病率高于普通对照组,高危干预组发病率低于高危对照组,普通干预组发病率低于普通对照组,差异均具有统计学意义（P〈0.05）。结论：母乳喂养6个月以上,4~6个月时添加辅食可以降低IE发病率,对脐血Ig E增高的高危儿干预效果更显著。Objective To evaluate the effects of cord blood IgE and feeding intervention on development of infant eczema （IE）. Method 60 infants whose umbilical cord blood IgE ≥ 1 IU/ml were defined as high - rish group, And other 60 infants whose umbilical cord blood IgE 〈 1 IU/ml were defined as normal group. The two groups were then respectively randomly assigned at birth to one of two dietary regimen protocols ; those in intervention group were breast fed till more than 4 months of age, then added solid foods during 4 - 6 months of age; and the control group accepted routine intervention. The cumulative incidence of infant eczema was investigated by a 12 - month follow - up. Results The incidence of IE was 30.0% （9/30） in the high - risk intervention group,73.3% （22/30） in the high - risk control group, 13.3% in the normal intervention group, and 33.3% in the normal intervention group. The results were respectively statistically significant. Conclusion Early feeding interventions which include breastfeeding, timely solid food introducing could reduce the IE incidence, both for high - risk and normal infants, and is probably an effective primary intervention method for allergic diseases of children.
