中药在慢性乙型肝炎治疗中的定位思考与研究

摘　　要：慢性乙型肝炎属于隐匿性、进展性、复杂性传染病,其病情复杂,预后不佳。乙型肝炎病毒载量是肝硬化和肝细胞癌发生的显著危险因素,过高的患病率也给家庭和社会带来了沉重的疾病负担。中医药治疗慢性乙型肝炎有着悠久的历史,开展了众多的干预研究,在临床上发挥着重要作用,包括缓解临床症状,改善肝功能,抑制病毒,调节机体免疫功能等多个方面。而抗病毒化学药物临床运用也存在一些问题(应答不佳、不良反应高发、耐药、停药复发),这给中药的应用提供了很好的契机。对疾病提出中西医联合的优化治疗策略和方案以及中药在其中的作用和定位,是开展中药现代药理学实验和循证医学临床研究的核心。通过将中医的辨证论治思维和西医的循证医学理念进行结合,进而探索中医药干预临床研究设计的基本思路,以期评价中医药/中西医联合治疗的整体优势、协同作用和差异化特点,为临床治疗决策的制定和中药的科学合理应用提供指导。Objective： The chronic hepatitis B is a concealed, progressive and complex contagion. H-epatitis B virus load is a notable risk factor for the occurrence of hepatic fibrosis and liver cancer. There was a heavy burden for the family and society. There were many intervention studies of traditional Chinese medicine treatment for chronic hepatitis B. Chinese medicine treatment had good results, which could relieve symptoms, improve liver function, inhibit virusand regulate immune system funciton. However, there were some problems of antiviral drugs＇ clinical application, including uncomplete answer, higher rate of adverse reaction, drug resistance and relapse after withdrawl. This provides a good opportunity for the application of traditional Chinese Medicine. Optimal treatment strategy and plan for Chinese and Western medicine were pivatol for the pharmacology experiment and clinical research.The design ideaof traditional Chinese medicine clinical research was explorated through the combination TCM of syndrome differentiation and treatment and evidence based medicine in Western Medicine. Based on these, there would be betterly used to evaluate the overall advantage, integral effect and differentiation characteristics of Chinese and Western medicine.These could provide guide for the clinical treatment decision and scientific and reasonable application of Chinese medicine.
