摘　　要：目的建立较为完善的阴式手术准入标准与风险预测模型,选择合适的患者实施经阴道手术。方法回顾性分析本院2013年1月~2015年5月期间,在我院妇科通过阴式手术,腹腔镜辅助下阴式手术,腹腔镜手术实施的全子宫切除（非脱垂）、卵巢囊肿剥除、子宫肌瘤剥除手术病例共184例,制定经阴道手术的风险预测模型并进行评估。结果风险预测模型评分≤6分是可以经阴道手术的,≤4分经阴道手术是非常安全的。风险预测模型评分≥4分,经腹腔镜手术或者腹腔镜辅助下阴式手术是可行的。结论对拟行阴式手术的患者术前通过手术风险预测模型进行评估,可充分估计手术难度,选择合适的患者进行阴式手术,最大程度的降低手术风险,保障手术安全。Objective To establish a relatively perfect accessory standards and risk prediction model of vaginal surgery,choosing appropriate patients for vaginal surgery. Methods A retrospective analysis of 184 patients from January 2013 to May 2015 received surgery in Gynecology Department of our hospital,involved of hysterectomy （non prolapse）,ovarian cystectomy,myomectomy by way of vaginal surgery,laparoscopic assisted vaginal surgery,laparoscopic surgery,we formulated the risk prediction model of vaginal surgery and evaluated the risk factors. Results The score of risk prediction model valued ≤ 6 points,patients could receive vaginal surgery.The score valued ≤ 4 points,it was safe to perform vaginal surgery.The score of risk prediction model valued ≥ 4 points,it was feasible for patients to receive laparoscopic assisted vaginal surgery or laparoscopic surgery. Conclusion Evaluation of patients are performed pre-operatively by the risk prediction model,we can fully estimate the difficulty of operation and select appropriate patients to perform vaginal surgery to reduce risk of surgery and ensure the safety of operation.
