摘 要:目的:对比静息、运动负荷核素心肌灌注显像(MPI)与超声心动图诊断心肌桥患者心功能的临床价值。方法:回顾性选取28例经多层螺旋血管造影(CTA)确诊的心肌桥患者,行核素心肌灌注显像及超声心动图检查,对比分析检查所见及诊断结论。结果:静息心肌灌注显像发现心肌桥心肌缺血性改变15例,阳性检出率为53.57%(15/28);运动负荷心肌灌注显像发现心肌缺血17例,阳性检出率为60.71%(17/28),静息与运动负荷心肌灌注显像对心肌桥患者心肌缺血改变的阳性检出率差异无统计学意义(x^2=0.292,P>0.05)。超声心动图共检出心肌缺血患者19例,阳性检出率为67.86(19/28);静息、运动负荷心肌灌注显像与超声心动图检查在评价心肌桥心肌缺血中差异无统计学意义(x^2=1.198,x^2=0.311,P>0.05)。结论:静息心肌灌注显像、运动负荷心肌灌注显像与超声心动图检查在评价心肌桥心肌缺血方面差异无明显统计学意义,联合超声心动图检查能够达到全面准确诊断心肌桥患者心功能情况的目的。Objective: To compare resting, exercise load nuclide myocardial perfusion imaging (MPI) and the clinical value of echocardiography in the diagnosis of myocardial bridge in patientswith cardiac function. Methods: Twenty eight patients with myocardial bridge were confirmed by multislice CTA line nuclide myocardial perfusion imaging and echocardiography. The results and diagnosis conclusion have been analyzed. Results: The results found that 15 cases had myocardial bridge resting myocardial perfusion imaging myocardial ischemic change, positive detection rate was 53.57%(15/28); 17 cases were found stress myocardial perfusion imaging myocardial ischemia, positive detection rate was 60.710/0(17/28), resting and stress myocardial perfusion imaging of myocardial ischemia in patients with myocardial bridge change positive detection rate difference has no statistical significance(x2=0.292, P〉0.05). Nineteen patients with myocardial ischemia were detected by supersonic and enchanted the graph, positive detection rate of 67.86(19/28). Resting myocardial perfusion imaging and stress myocardial perfusion imaging and echocardiography in the evaluation of myocardial ischemic myocardial bridge were no significant differences(x2=1.198, x2=0.311, P〉0.05). Conclusion: There were no significant differences in the assessment of myocardial ischemia myocardial bridge within resting myocardial perfusion imaging, stress myocardial perfusion imaging and echocardiography, the joint echocardiography can achieve the purpose of full and accurate diagnosis of cardiac function in patients with myocardial bridge.
