摘 要:目的通过《抗菌药物临床应用管理办法》(以下简称《办法》)的实施,规范基层医院外科I类切口手术患者围手术期抗菌药物预防使用。方法选择《办法》实施前的2011年1~12月我院外科全部I类切口手术患者2693例和实施后的2014年1~12月我院外科全部I类切15手术患者2973例,回顾性分析其同手术期抗菌药物预防使用率、品种选择合理率、预防使用时间符合率等因素对I类手术切口感染率的影响。结果《办法》实施前、后抗菌药物使用观察指标分别为:抗菌药物预防使用率由92.87%下降到37.13%.差异有统计学意义(P〈0.05)。品种选择合格率由77.1.0%上升到80.97%,差异无统计学意义(P〉0.05)。时间符合率由20.41%上升到63.49%,差异有统计学意义(P〈0.05)。切口感染率分别为0.48%和0.47%,差异无统计学意义(P〉0.05)..结论《办法》实施后预防使用抗菌药物比例显著降低,时间符合率上升,而外科I类手术切口感染率并没有上升。Objective To implement the management of the clinical application of antibacterial drugslu, "measures" for short, and to investigate the prevent use of antibiotics in the perioperative period of surgical patients in primary hospital. Methods In the hospital, before and after the implementation of the "measures", 2693 cases of surgical patients with type I incision from January to December in 2011 and 2973 cases of surgical patients with type I incision from January to December in 2014 were recruited, respectively. The percent of prevent use of antibacterial drugs, the reasonable se- lection of antibacterial drugs and time compliance rate of antibacterial drugs were retrospectively analyzed. Results Before and after the implementation of the "measures", the percent of prevent use of antibacterial drugs was decreased from 92.87% to 37.13%, the difference was statistically significant(P〈0.05). The reasonable selection of antibacterial drugs was increased from 77.10% to 80.97% and the difference was statistically significant(P〈O.05). Time compliance rate of antibacterial drugs was increased from 20.41% to 63.49%, the difference was statistically significant (P〈O.05). Incision infection rate was 0.48% and 0.47% respectively, the difference was not statistically significant(P〉0.05). Conelusion After the implementation of the "measures", the percent of prevent use of antibacterial drugs was significantly decreased, time compliance rate of antibacterial drugs was increased significantly, and surgical incision infection rate has not risen.
