摘 要:目的:研究改良换血术在重症新生儿高胆红素血症治疗中的疗效及安全性。方法:对17例符合换血指征的重症新生儿高胆红素血症患儿采取手动控制密闭动脉抽血,静脉微量泵输血外周动静脉同步换血的改良换血术进行治疗。检测换血前后患儿血清胆红素、血糖、血常规、呼吸、心跳数据,采用SPSS17.0统计软件进行数据分析。结果:17例换血术中无堵管现象,换血前后总胆红素、非结合胆红素、结合胆红素下降明显,白细胞、血小板减少,血糖升高,换血前后差异有统计学意义(P<0.05)。呼吸、心跳、血红蛋白比较,换血前后差异无统计学意义(P>0.05)。结论:改良换血术在新生儿高胆红素血症治疗中,胆红素换出率高,对患儿内环境无影响,并发症少,安全,值得开展。Objective To study the clinical effect and safety of of improved exchange transfusion in the treatment of severe neonatal hyperbilirubinemia. Method Seventeen neonates with severe neonatal hyperbilirubinemia who met the indications of exchange transfusion in my hospital were treated with improved exchange transfusion therapy of peripheral aiteries and veins by drawing blood manual control and closed in aiteries,blooding transfusion with micro pump in veins. The serum bilirubin,glucose,blood routine,breathing and heart rate were measured before and after exchange transfusion. They were analysed with spss 17. 0. Results Seventeen cases were not blocking tube. The serum total bilirubin,indirect bilirubin,direct bilirubin,leukocytes and platelets levels decreased significantly,glucose increased significantly dafter blood exchange therapy,there were statistically signficant difference( P〈0. 05). there were no statistically signficant difference in breathing,heart rate and hemoglobin dafter blood exchange therapy( P〉0. 05). Conclusion The swap out rate of bilirubin is higer with improved exchange transfusion in the treatment of severe neonatal hyperbilirubinemia. The therapy is no influence on the internal environmet of neonates. The therapy is worthy of promoting,because it is no complication and security.
