降钙素原在手足口病中的临床意义

摘　　要：目的探讨降钙素原（PCT）水平在手足口病诊疗中的临床意义。方法回顾性分析2014年3月-2015年8月住院的98例手足口病患儿的临床资料,分析手足口病的临床特点,比较普通型组和重症组,单纯病毒感染组、合并细菌感染组和重症组的PCT水平,并分析PCT与其他指标的相关性。结果重症组PCT值高于普通型组,差异有统计学意义（P〈0.05）,重症组中死亡病例的PCT值明显高于该组平均水平,单纯病毒感染组、合并细菌感染组和重症组三组的PCT比较,差异无统计学意义（P〉0.05）,但呈现逐渐升高的趋势。PCT与白细胞计数（WBC）、乳酸脱氢酶（LDH）呈正相关,差异有统计学意义（P〈0.05）,PCT与中性粒细胞数（NE）、中性粒细胞率（NE%）、血小板计数（PLT）、血小板平均体积（MPV）不相关（P〉0.05）。结论 PCT在鉴别手足口病合并细菌感染、早期识别重症手足口病方面有一定临床价值,应结合临床资料综合分析。Objective To evaluate the clinical significance of procalcitonin（PCT）in diagnosis and treatment of hand,foot and mouth disease. Methods Clinical data of 98 HFMD children who were hospitalized between March 2014 and August 2015 were analyzed retrospectively to analysis the clinical manifestation of HFMD,the PCT level of normal group and severe group,simple virus infection group,viral associated with bacterial infection group and severe group were compared,in addition, the relationships between PCT and other measurements were analyzed statistically. Results The PCT level of severe group was higher than normal group,the difference was significant statistically（P〈0.05）,and the PCT values of death cases in severe group were obviously higher than the average level,there was no statistically significant difference in the level of PCT among simple virus infection group, viral associated with bacterial infection group and severe group（P〉0.05）,but which showed a rising trend.The PCT has notable positive relationship with WBC,LDH（P〈0.05）,however,it＇s no correlation with NE、NE%、PLT、MPV（P〈0.05）. Conclusion The level of PCT has certain value for the diagnosis of HFMD with bacterial infection and early recognition of severe HFMD,which should be analysed synthetically combined with clinical data.
