摘  要:目的:动态观察创面应用丝素膜真皮支架对创面修复过程中成纤维细胞(Fb)功能的影响。方法:50只雄性SD大鼠,随机分为聚乙烯醇海绵组(海绵组)和丝素膜组(n=25)。于大鼠背部正中切取皮肤制成2 cm×2 cm全层皮肤缺损创面,分别在创面埋植聚乙烯醇海绵(海绵组)或丝素膜(丝素膜组),再覆以自体刃厚皮缝合。术后1、2、3、4、6周取移植物及其周围组织,HE染色观察其组织形态学变化,免疫组化法检测组织α平滑肌肌动蛋白(α-SMA)和转化生长因子-β1(TGF-β1)的表达,TUNEL法检测细胞凋亡水平。结果:光镜观察发现,术后1~2周,丝素膜组肉芽组织化较海绵组快,胶原纤维较密集;术后6周,与海绵组相比,丝素膜组胶原纤维较粗大,组织相对成熟。术后组织中α-SMA及TGF-β1表达增加,α-SMA表达量于术后2周达到高峰,之后逐渐下降。丝素膜组α-SMA表达量低于海绵组,在第1、6周与海绵组比较差异有显著性(P〈0.01);海绵组TGF-β1表达量于术后2周达到高峰,丝素膜组于术后第3周达高峰,除第3周外,丝素膜组TGF-β1表达量均低于海绵组,在第1、2、6周与海绵组比较差异有显著性(P〈0.01)。丝素膜组细胞凋亡率在术后各时相点均显著高于海绵组(P〈0.01)。结论:丝素膜与自体刃厚皮复合移植后,可不同程度减少Fb中α-SMA及TGF-β1的表达,并促进细胞凋亡,加快组织重塑,有利于组织修复的完成,对防止瘢痕过度形成具有积极意义。Objective: To dynamically investigate the effect of silk fibroin membrane as dermal scaffold on the fibroblast behaviors in wound healing. Methods: Fifty male SD rats with a full-thickness wound the size of 2 cm×2 cm on the back skin were randomly divided into 2 groups. The silk fibroin membrane group was covered by silk fibroin membrane with overlying thin split-thickness autograft, while that of the sponge group was covered by a PVA sponge and split-thickness autograft as control. Wound specimens were obtained at 1st, 2nd, 3rd, 4th, and 6th week post operation. Histological changes of wound tissues were observed with HE staining, and α-smooth muscle actin(α-SMA) and expression of transforming growth factor-β1(TGF-β1) were detected by immunohistochemical staining, and cell apoptosis was revealed by TUNEL assay. Results: HE staining showed faster formation of granulation tissue and denser collagen fiber in silk fibroin membrane group than in sponge group 1-2 weeks after surgery. Six weeks after operation, the collagen fiber in the silk fibroin membrane group was thicker and the newly-formed tissue was relatively mature as compared with that of the sponge group. The expression of α-SMA and TGF-β1 was increased gradually during wound healing in both groups, and α-SMA expression reached a peak in 2nd week, and then it decreased. The silk fibroin membrane group showed a lower expression of α-SMA than that of the sponge group, and the difference was markedly significant at 1st and 6th week(P0.01). The expression of TGF-β1 reached a peak at 2nd week in sponge group, whereas the peak appeared in 3rd week in silk fibroin membrane group, and it was significantly lower when compared with that of sponge groups at 1st, 2nd and 6th week(P0.01). The percentage of cell apoptosis was significantly higher in silk fibroin membrane group during the designed experimental time when compared with sponge group(P0.01).
