摘 要:目的为椎体成形术后对邻近椎体生物力学影响的实验研究制作一种中间椎体压缩性骨折的体外骨质疏松五联椎体模型。方法采用新鲜成年猪脊柱胸腰段五联椎体分为A、B、C、D 4组,每组15个。A组置于0.9%生理盐水中不经脱钙处理;B组置入0.491 6 mmol/L的EDTA-Na2脱钙液中浸泡脱钙4周;C组通过打孔的中间椎体椎弓根推注脱钙液脱钙3周后,再整体置入脱钙液中浸泡3周;D组通过打孔的中间椎体椎弓根推注脱钙液脱钙3周后,将中间椎体制成Ⅱ度压缩骨折,再整体置入脱钙液中浸泡3周。对所有的单元椎进行骨密度(BMD)值和CT值比较,DR片及大体观察。结果 B组椎体经脱钙后较A组BMD值及CT值下降明显,且B组椎体第3周与第4周BMD值与CT值变化差异无统计学意义(P>0.05);B组与C组中间的椎体在相同的时间里经过BMD值及CT值的比较差异无统计学意义(P>0.05);第4周检测的D组中间压缩骨折的椎体与B、C组中间椎体对比BMD值及CT值差异无统计学意义(P>0.05);随着脱钙时间的延长,DR片显示透亮区增大,大体观察显示骨小梁断裂及消失逐渐增多。结论采用0.491 6 mmol/L的EDTA-Na2对成年猪胸腰段五联椎体进行脱钙,制成中间椎体压缩骨折的骨质疏松性五联椎体模型,可以作为研究经皮椎体成形术后邻近椎体的骨折的一种标准化模型。Objective To make a model of five osteoporotic vertebral that intermediate vertebral compression fractures, to be used for experimental studies of biomechanical influence after vertebroplasty. Methods Fresh adult porcine thoracolumbar vertebrae were used in this study, which were divided into 4 groups, each of 15. Group A was placed in 0.9% saline without decalcification. Group B was soaked 4 weeks in 0.491 6 mmol/L of EDTA-Na2 for decalcification. Group C operation was as follows, decalcification solution was inserted into perforated intermediate vertebral pedicle, then put into 0.491 6 mmol/L of EDTA-Na2 after 3 weeks. Group D according to the group C, but before the model put into decalcification solution, intermediate vertebral was made into Ⅱ degree compression fractures. BMD, CT values, DR and appearance were compared in every vertebral. Results BMD, CT values of Group B had no difference between 3 and 4 weeks respectively(P〈0.05), they were both significantly lower than Group A(P〈0.05). BMD, CT values of Group B and C had no differences in the same time(P〈0.05). BMD, CT values of intermediate vertebral that compression fractures of Group D had no differences with Group B and C(P〈0.05). Bright area in DR, trabecular bone fracture and disappear were all increased as the extension of decalcified time. Conclusions Making a model of five osteoporotic vertebral that intermediate vertebral compression fractures using adult porcine thoracolumbar vertebrae which were decalcified by 0.491 6 mmol/L of EDTA-Na2, it can be used as a standardized model for research of adjacent vertebral fractures after percutaneous vertebral.
