神经外科患者围手术期抗菌药物应用时间的调查分析

摘　　要：目的探讨神经外科患者围手术期预防性使用抗菌药物的时间分布,评价其合理性。方法抽取2013年1月至2015年1月8家医院神经外科手术患者共2 400例,其中三级甲等医院4家、二级甲等医院4家,自行设计《抗菌药物应用时间调查表》,采集患者病历中应用抗菌药物名称,术前抗菌药物应用时间,术中是否追加抗菌药物,术后停用抗菌药物时间等信息。结果 2 400例神经外科患者中,Ⅰ类切口1 308例,Ⅱ类切口1 092例,抗菌药物预防性使用率为100%,合计使用抗菌药物11类35种,使用例次为6 314例次,557例次(7.74%)患者的使用时间不合理。结论需加强神经外科患者围手术期预防性使用抗菌药物的用药监测,促进合理用药。Objective To detect the application time of antibacterial drugs by neurosurgical patients with surgical operation during perioperative period, and to increase the level of rational drug use. Methods Data of 2 400 neurosurgical patients undergoing operation were randomly selected from January 2013 to January 2015 from 8 hospitals, 4 hospitals were grade-three and class-A, 4 hospitals were grade-two and class-A. The questionnaire of application time for antimicrobial drugs was used, and the information of application time of antibacterial drugs was analysed retrospectively. Results The situation of the surgical patients was as follows： 1 308 cases were class Ⅰ surgical incision, 1 092 cases were class Ⅱsurgical incision. Ratio of antibacterial drugs use was 100%, types of antibiotics were 35, 6 314 cases used antibiotics, ratio of irrational application time of antibacterial drugs was 7.74%. Conclusion Management of application time of antibacterial drugs for neurosurgical patients with surgical operation should be strengthened.
