中年高尿酸血症人群有效控制尿酸的自我管理模式研究

摘　　要：目的探讨应用自我管理模式对中年高尿酸血症（HUA）人群疾病认知及血尿酸的影响,为提高健康水平提供参考。方法选择2014年4月-2015年9月在右江民族医学院附属医院内科门诊就诊,符合高尿酸血症的患者98例,给研究对象发放自制的问卷表进行调查,6个月后对患者应用自我管理模式前后对疾病认知、自我管理依从性及血尿酸控制指标进行自身对照。结果应用自我管理模式后患者对疾病认知水平和自我管理依从性较管理前明显提高（P〈0.001）,6个月后痛风发作次数、口服用药次数、静脉用药次数较6个月前明显减少（P〈0.001）,血尿酸控制有效率为82.6%,较管理之前的35.7%明显提高（P〈0.001）。结论采取自我管理模式对患者进行管理,能促进患者掌握疾病相关知识,增强自我效能感,有效控制尿酸,提高健康水平。Objective To explore the influence of self-management model toward the disease cognition and blood uric acid level of middle-aged hyperuricemia（ HUA） patients,so as to provide reference for improving their health status. Methods 98 HUA patients admitted to Internal Medicine Department of Affiliated Hospital of Youjiang Medical University for Nationalities from April,2014 to September,2015 were selected and given the self-made questionnaires. Six months later,they were given self-management model again to compare disease cognition,compliance of self-management and control index of blood uric acid before and after self-management were compared. Results After self-management,the levels of disease cognition and compliance of self-management significantly improved while comparing to those before self-management（ P〈0. 001）. Six months later,attacks of metabolic arthritis,oral medication times and vein medication times were apparently less than those six months ago（ P〈0. 001）. The effective rate（ 82. 6%） of blood uric acid control was significantly higher than that（ 35. 7%） before self-management（ P〈0. 001）. Conclusion Self-management model can improve HUA patients＇ cognition of disease,increase their self-efficacy,control their uric acid effectively and improve their health status.
