多元化健康教育对涤纶套双腔导管维持性血透患者导管功能不良的影响

摘　　要：目的探讨多元化教育模式对留置涤纶套双腔导管行维持性血透患者导管功能不良(导管失功)的影响。方法将100例留置涤纶套双腔导管行维持性血透的患者随机分为研究组和对照组各50例;对照组实施传统的健康教育,研究组应用多元化健康教育模式进行干预;观察干预前后两组疾病相关知识与导管维护技能的掌握情况;比较干预后6个月内两组的导管失功率。结果干预前两组疾病相关知识与导管维护技能评分差异无统计学意义(P=0.2062),干预后两组疾病相关知识与导管维护技能评分均较干预前提高(P<0.001),且研究组提高幅度大于对照组(P<0.001);干预后6个月内研究组和对照组导管失功率分别为12.00%、28.00%,两组比较差异有统计学意义(P=0.0455)。结论多元化健康教育模式能更大程度地提高留置涤纶套双腔导管行维持性血透患者疾病知识和导管维护技能水平,提高导管自我护理的能力,降低导管失功率,值得临床推广应用。Objective To explore effects of diversified health education on indwelling catheter dysfunction（ fistula dysfunction） of maintenance hemodialysis patients by dual-lumen catheter with a Dacron cuff. Methods 100 maintenance hemodialysis patients with indwelling catheter dysfunction by dual-lumen catheter with a Dacron cuff were randomly divided into research group and control group with 50 cases in each. The control group were given traditional health education,while the research group were intervened by diversified health education. Then,mastery of disease-related knowledge and skills of catheter maintenance before and after intervention in both groups were observed. In addition,probability of catheter dysfunction within 6 months after intervention were compared between two groups. Results Before intervention,difference of scores of disease-related knowledge and skills of catheter maintenance was not statistically significant（ P = 0. 2062）. After intervention,the above-mentioned scores of both groups all improved（ P〈0. 001）,and those of the research group improved more significantly than those of the control group（ P〈0. 001）. After intervention,probability of catheter dysfunction of the research group and control group was 12. 00% and 28. 00%,respectively,and the difference between the two groups was statistically significant（ P = 0. 0455）. Conclusion Diversified education mode can further improve mastering levels of diseaserelated knowledge and catheter maintenance skills of maintenance hemodialysis patients by dual-lumen catheter with a Dacron cuff,improve the ability of catheter self-care,and reduce the probability of catheter dysfunction. Thus,it is worthy of popularization and application in clinical practice.
