摘  要:目的检测特发性血小板减少性紫癜(idiopathic thrombocytopenicpurpura,ITP)患者外周血B1细胞水平,观察、分析对其预后的影响,为临床诊治提供依据。方法根据治疗结果将59例ITP患者(观察组)分为有效组(34例)和无效组(25例),选取同期体检正常人群61例作为对照组。检测外周血血小板(PLT)计数,CD19+细胞、B1细胞、血小板相关免疫球蛋白(PAIg G)等。结果观察组PLT低于对照组,B1淋巴细胞、CD19+细胞和PAIg G均高于对照组,差异有统计学意义(P<0.05)。治疗后,患者PLT高于治疗前,B1淋巴细胞、CD19+细胞和PAIg G均低于治疗前,差异有统计学意义(P<0.05);无效组患者治疗后PLT低于有效组,B1淋巴细胞、CD19+细胞和PAIg G均高于有效组,差异有统计学意义(P<0.05)。结论 ITP患者外周血PLT显著降低,B1淋巴细胞、CD+19细胞和PAIg G显著升高,患者治疗效果不佳。Objective To detect the level of peripheral blood B1 cells of patients with idiopathic thrombocytopenic purpura( ITP) and to analyze its implication for the prognosis of the patients. Method 59 ITP patients were firstly chosen according to the treatment results as the experimental group of the study,where the treatment of 34 cases was effective while that of the other 25 was ineffective. In the mean time,61 people of sound health were chosen as the control group of the study.Then the peripheral blood platelet( PLT) count,CD19+cells,B1 cells,platelet associated immunoglobulin( PAIgG) of both groups were detected. Findings The PLT of the experimental group was lower than that of the control group. The difference is statistically significant( p 〈0. 05). The B1 cells,CD19+cells and PAIgG of the experimental group were higher than those of the control group. The difference is statistically significant( p 〈0. 05). The PLT of the patients as detected after treatment was higher than that before treatment. The level of B1 cells,CD19 cells and PAIgG of the patients as detected after treatment was lower than that before treatment. The difference was statistically significant( p 〈0. 05).After treatment,the PLT count of the patients of ineffective treatment was lower than that of the patients of effective treatment. The level of B1 lymphocyte,CD19+cells and PAIgG of the patients of ineffective treatment was higher than that of the patients of effective treatment. The difference was statistically significant( p 〈0. 05). Conclusion The peripheral blood PLT of the ITP patients decreased considerably and the level of B1 lymphocytes,CD19 + cells and PAIgG significantly increased,which implies ineffective treatment.
