摘 要:目的探讨高海拔地区头颈部放疗对口腔内环境的影响及防治措施。方法分析青海省第五人民医院2014年1月~2015年11月收治的120例头颈部放疗患者临床资料,依据是否实施早期预防措施进行分组,对照组(放疗措施)60例和实验组(放疗措施+预防措施)60例。观察两组放疗前后刺激性唾液流量、唾液pH值、唾液成分情况,观察两组放疗后口腔黏膜炎和放射性龋情况。结果两组放疗前刺激性唾液流量、唾液pH值、钠离子(Na^+)、钾离子(K^+)、氯离子(Cl^-)、磷离子(P^(3-))、唾液淀粉酶浓度比较,差异无统计学意义(P>0.05)。对照组放疗后刺激性唾液流量、唾液pH值、Cl^-、P^(3-)、唾液淀粉酶浓度均低于放疗前,Na^+高于放疗前,实验组放疗后刺激性唾液流量、唾液淀粉酶浓度均低于放疗前,实验组放疗后刺激性唾液流量、唾液pH值、Cl^-、P^(3-)、唾液淀粉酶浓度均高于对照组,Na^+低于对照组,差异均有统计学意义(P<0.05)。实验组放疗后口腔黏膜炎和放射性龋发生率均低于对照组,差异有统计学意义(P<0.05)。结论放疗措施+预防措施在高海拔地区头颈部放疗患者中应用,可以改善口腔内环境,降低并发症发生率,值得临床推广应用。Objective To explore the influence and prevention of PATE radiotherapy on mouth environment in high altitude localities. Methods Clinical data of 120 patients with PATE radiotherapy in the Fifth People's Hospital of Qinghai Province from January 2014 to November 2015 were analyzed. Patients were divided into two groups by early prevention measure, control group (radiation treatment measure) was 60 cases and experiment group (radiation treatment measure + prevention measure) was 60 cases. The stimulating saliva flow, saliva pH, salivary components of two groups before and after radiotherapy were detected, the oral mucositis and radioactive decay of two groups after radiotherapy were detected. Results The stimulating saliva flow, saliva pH, Na^+, K^+, Cl^-, P^3-, saliva amylase concentrations of twd groups before radiotherapy were compared, there were no statistical differences (P 〉 0.05), The stimulating Saliva flow, saliva pH, Cl^-,P^3-, saliva amylase concentrations of control group after radiotherapy were lower than those before radiotherapy, Na^+ was higher than that before radiotherapy; stimulating saliva flow, saliva amylase concentrations of experiment group after radiotherapy were lower than those before radiotherapy, the stimulating saliva flow, saliva pH, Cl^- P^3-, saliva amylase concentrations of experiment group after radiotherapy were higher than those of control, group, Na^+ was lower than that of control group, with statistical differences (P 〈 0.05). The incidence rate of oral mucositis and radioactive decay of experiment group after radiotherapy were lower than those of control group, the differences were statistically significant (P 〈 0.05). Conclusion The radiation treatment measure + prevention measure applied in PATE radiotherapy in high altitude localities can improve mouth environment, decrease incidence rate of complication, it is worthy of clinical promotion and application.
