滤泡辅助性T细胞在自身免疫性溶血性贫血发病机制中的作用

摘　　要：目的研究自身免疫性溶血性贫血(autoimmune hemolytic anemia,AIHA)患者外周血滤泡辅助性T细胞(T follicular helper cells,Tfh细胞)数量的变化,探讨Tfh细胞在AIHA发病机制中的作用。方法采用流式细胞术的方法检测分析26例AIHA患者及30例正常人(对照组)外周血B细胞、成熟B细胞及CD4+T细胞比例,Tfh细胞数量。结果 AIHA患者外周血CD19+B细胞百分比为(17.46±16.65)%,高于对照组(9.12±4.60)%(P=0.011);成熟B细胞百分比为(87.33±7.80)%,明显高于对照组(76.52±13.90)%(P=0.001)。AIHA患者CD4+T细胞为(25.47±11.75)%,较正常对照组(34.36±7.21)%明显减低(P=0.001)。AIHA患者Tfh细胞比例为(6.72±5.38)%,明显高于对照组(1.52±1.05)%(P=0.000)。结论 Tfh细胞数量的扩增参与了AIHA病理过程,并在其发病过程中起重要作用。Objective To investigate the changes of follicular helper T cells in peripheral blood of patients with Autoimmune hemolytic anemia（ AIHA）,to explore the role of Tfh cells on the pathogenesis of AIHA. Methods The peripheral blood B cells,mature B cells and CD4＋T cells ratio,and the number of Tfh cells in 26 cases of AIHA patients and 30 healthy controls were detected by flow cytometry. Results The CD19＋B cells in the patients with AIHA（ 17. 46 ± 16. 65%） were significantly higher than that of healthy controls（ 9. 12 ±4. 60%）. The mature B cells in the patients with AIHA（ 87. 33 ± 7. 80%） were significantly higher than healthy controls（ 76. 52 ± 13. 90%）. The CD4＋T cells in AIHA patients（ 25. 47 ± 11. 75%） were obvious less than healthy controls（ 34. 36 ± 7. 21%）. The Tfh cells in patients with AIHA（ 6. 72 ± 5. 38%） were significantly more than healthy controls（ 1. 52 ± 1. 05%）. Conclusion The expansion of Tfh cells may involve in AIHA pathological process and play an important role in the process of its pathogenesis.
