环磷酰胺联合泼尼松治疗老年性SLE对患者血清各指标影响及其疗效观察

摘　　要：目的分析环磷酰胺联合泼尼松治疗老年性SLE对患者血清各项指标的影响,并探讨其临床疗效及安全性。方法选取我院于2012年4月至2015年4月收治的64例老年性SLE患者,在患者自愿的前提下随机分成对照组(泼尼松治疗)和观察组(环磷酰胺联合泼尼松治疗),每组32例。检测治疗前后两组患者血清免疫球蛋白A、G、M(Ig A、Ig G、Ig M)、补体C_3、C_4及抗ds-DNA抗体水平变化情况,分析两种治疗方法对上述各指标的影响,并对比两组患者的疗效及治疗期间不良反应发生情况。结果治疗3个月后两组患者血清Ig A、Ig G、Ig M及抗ds-DNA抗体水平显著低于治疗前(P<0.05),补体C_3、C_4水平显著高于治疗前(P<0.05);且观察组血清各免疫球蛋白、抗ds-DNA抗体水平降低程度及补体C_3、C_4水平升高程度显著大于对照组(P<0.05);同时观察组治疗总有效率显著高于对照组(P<0.05);治疗期间,两组患者不良反应情况比较无显著差异(P>0.05)。结论环磷酰胺联合泼尼松治疗老年性SLE能显著降低血清各免疫球蛋白及抗ds-DNA抗体水平,同时升高补体C_3、C_4水平;且其疗效显著,安全可靠,值得在临床上推广应用。Objective To explore the effect of cyclophosphamide combined with prednisone treatment on serum indexes in aged patients with SLE,and to explore the clinical efficacy and safety. Methods 64 cases of elderly patients with SLE were randomly divided into control group（ prednisone treatment） and the observation group（ cyclophosphamide combined with prednisone treatment）,32 cases in each group. The changes of serum immunoglobulin A,G,M（ Ig A,M,G）,C3,C4 and anti ds-DNA antibodies were detected in two groups.Results The levels of serum Ig A,C4,Ig M and anti ds-DNA antibody in both group after three month treatment were significantly lower than those before treatment（ P 〈 0. 05）,and serum levels of Ig G and C4 were significantly higher than those in control group（ P 〈 0. 05）. The total effective rate in the observation group was significantly higher than in control group（ P 〈 0. 05）,while the adverse effects rate in both groups had no significantly difference（ P 〉 0. 05）. Conclusion Cyclophosphamide combined with prednisone in the treatment of elderly patients with systemic lupus erythematosus（ SLE） can significantly reduce the serum immunoglobulin and anti DS DNA antibody levels,and increase the levels of complement C3 and C4. The effect is significant,safe and reliable,and may be worth in the clinical application.
