支气管镜下冷冻治疗支气管结核的疗效分析

摘　　要：目的探讨电子支气管镜下冷冻治疗支气管结核的疗效及安全性。方法选择2014年1月至2015年10月,收治于第四军医大学唐都医院呼吸内科并已确诊为支气管结核的患者共105例,随机分为对照组48例和治疗组57例。对照组给予常规全身化疗;治疗组在全身抗结核治疗基础上给予患者局部冷冻治疗。观察两组患者1个月后治疗总有效率、气促评分及并发症情况。结果经过1个月的治疗,冷冻治疗组总有效率(91.23%)明显好于对照组(71.91%)。治疗前对照组和治疗组患者气促评分相近(1.54±1.01 vs.1.46±1.02,P>0.05);治疗后,两组患者气促症状均较前明显改善,对照组和冷冻治疗组(1.02±0.57 vs.0.91±0.41,P<0.05)结果具有统计学意义。结论电子支气管镜下冷冻治疗支气管结核效果明显,安全、可靠,并发症少,预后好。Objective To observe the efficacy and safety of 105 cases of patients with endobronchial tuberculosis treated by bronchoscopic cryotherapy. Methods From January 2014 to October 2015 in Tangdu Hosptial respiratory department,chose the 105 patients diagnosed as endobronchial tuberculosis,and randomly divided into two groups,48 cases in control group,57 cases in treatment group. The control group received conventional systemic chemotherapy,and the treatment group was treated with local freezing treatment on the basis of systemic chemotherapy. The total efficiency,shortness of breath score and complications of patients were collected after 1 months of treatment. Results After one month of treatment,the total effective rate of the treatment group（ 91. 23%） was significantly higher than the control group（ 71. 91%）. Before treatment,the shortness of breath score of the treatment group and the control group of patients were similar（ 1. 54 ± 1. 01 vs.1.46±1. 02,P 〉0. 05）. After treatment,the shortness of breath symptoms of two groups were significantly improved,especially for the treatment group（ 1.02±0.57 vs. 0.91±0.41,P〈0.05）. Conclusions The effect of treatment of bronchial tuberculosis with bronchoscopic cryotherapy is obvious, safe and reliable, the complications are few,and the prognosis is good.
