初次分娩妇女阴道分娩的产程曲线研究

摘　　要：目的:探讨初次分娩妇女阴道分娩的产程曲线特点和变化。方法:回顾性分析2013年1月至2014年12月期间在本院住院治疗的初次分娩并阴道分娩产妇共1640例,将患者分为2013年组和2014年组,比较两组患者的一般资料,统计分析两组患者的差异,探讨影响产程的因素,并将产妇分为应用缩宫素组和未用缩宫素组,并绘制缩宫素组、未用缩宫素组以及总例数产妇的产程曲线图。结果:2013年组共有产妇806例,2014年总例数为834例,两组患者的平均体重指数、平均孕期、平均新生儿体重,巨大婴儿的比例的差异均无统计学意义;2014年组患者的平均年龄,产后出血的发生比例,分娩镇痛的应用比例,产钳助产的比例,以及引产的比例均显著的高于2013年组;本研究中记录到的相关产程数据的患者共1546例,以经历时间为横坐标,以宫口扩张程度为纵坐标做产程曲线图,可以发现缩宫素组、未用缩宫素组以及总例数三组患者的产程曲线均呈缓慢上升趋势。结论:近两年的产程曲线已经与几十年前发生了变化,具有了新的特点,产程曲线呈缓慢上升的趋势,缺乏明显的减速期和加速期,这些变化对于重新定义我国国内的产程曲线具有重要意义。Objective： To investigate the characteristics and changes of the curve of labor duration of vaginal delivery women with initial delivery. Method： 1640 cases of vaginal delivery women with initial delivery in our hospital from Jan. 2013 to Dec. 2014,were retrospectively analyed and divided into the group of2013 year and the group of 2014 year,the general data of two groups were compared,the differences of two groups were statistically analyzed,and the factors affecting the labor duration were analyzed,then the women were divided into the oxytocin group and the non- using oxytocin group,and the curve of labor duration of oxytocin group,the non- using oxytocin group and the total number of patients were drawn. Result： 806 puerperae were in the group of 2013 year,and 834 puerperae in the group of 2014 year,Average body mass index,mean pregnancy,average weight of newborn babies,and the proportion of large infants of the two groups showed no significant difference; The average age of patients,incidence of postpartum hemorrhage,the proportion of application of labor analgesia,forceps delivery ratio,and the proportion of labor in the group of 2014 year were significantly higher than those of the group of 2013 year; the related data of labor duration of the total 1546 puerperae were recorded,the curve of labor duration taking the time as the horizontal axis,the degree of the palace mouth expansion as the longitudinal axis,showed a slow upward trend in all three oxytocin groups,the non- using oxytocin group and the total number of patients group. Conclusion：The curve of labor duration in the past two years has changed,with new features,the curve is slowly rising without obvious deceleration and acceleration period. These changes have important significance for the redefinition carve of labor duration of our country.
