C反应蛋白白细胞介素-6及白细胞计数在足月胎膜早破宫内感染中的意义

摘　　要：目的:探讨C反应蛋白(CRP)、白细胞介素-6(IL-6)及白细胞(WBC)计数联合检测对足月胎膜早破(PROM)孕妇宫内感染的意义。方法:回顾性分析我院足月PROM孕妇60例,按破膜时间分为>24h组和≤24h组,每组各30例,另选取同期我院正常足月产妇30例为对照组。检测患者CRP、IL-6水平和WBC计数,分析其与PROM孕妇宫内感染的关系。结果:PROM孕妇CRP、IL-6水平及WBC计数均显著高于对照组(P<0.05);破膜时间>24h组CRP、IL-6水平及WBC计数均高于破膜时间≤24h组(P<0.05);应用抗生素24h时PROM孕妇CRP、IL-6水平及WBC计数均高于入院时和应用抗生素72h时水平(P<0.05)。结论:CRP、IL-6及WBC联合检测对PROM孕妇早期宫内感染的预测与诊断有重要意义,为临床合理使用抗生素治疗提供理论依据。Objective： To investigate the significance of combined detection of CRP,IL- 6 and WBC count in the intrauterine infection of PROM pregnant women. Method： Retrospectively analysed 60 cases of term PROM pregnant women in our hospital,according to the film breaking time,were divided into〉 24 h group and ≤24h group,30 cases in each group. Another 30 cases over the same period in our hospital normal full- term parturient woman were selected as the control group. CRP,IL- 6 level and WBC count were detected,and its influence on PROM intrauterine infection was analyzed. Result： The levels of CRP,IL- 6and WBC in PROM were significantly higher than those in the control group,the difference was significant（ P〈0. 05）; The CRP,IL- 6 level and WBC count of patients with rupture of membrane in the 24 h group was higher than that in the ≤24h group,and the count of the〉 24 h group were higher than that of the≤24h group,the difference was significant（ P〈0. 05）; The levels of CRP,IL- 6 and WBC were higher in the PROM pregnant women at the time of application of 24 hours,than those of 72 hours,the difference was significant（ P〈0. 05）. Conclusion： The combined detection of CRP,IL- 6 and WBC has important significance for the prediction and diagnosis of early intrauterine infection of PROM in pregnant women,which provides theoretical basis for clinical rational use of antibiotics.
