摘 要:目的研究适量饮用白酒对胰岛素治疗的2型糖尿病患者氧化应激和炎症的急性影响。方法选取2012年5月—2013年8月于南京市第一医院内分泌科住院予胰岛素泵治疗符合纳入与排除标准的2型糖尿病患者40例为研究对象。患者入院后行胰岛素泵治疗,禁酒结束第1天与第2天17:30随晚餐分别饮用等量的矿泉水和白酒,饮用矿泉水和白酒者次日检测丙二醛(MDA)、可溶性细胞间黏附分子1(s ICAM-1)水平。结合动态血糖监测的结果,根据患者饮酒后24 h是否出现低血糖分为无低血糖组(25例)和低血糖组(15例)。结果患者饮水后MDA、s ICAM-1水平与饮酒后比较,差异无统计学意义(P>0.05)。低血糖组患者饮水后MDA水平低于无低血糖组(P<0.05);低血糖组患者饮酒后MDA水平、饮水后s ICAM-1水平、饮酒后s ICAM-1水平与无低血糖组比较,差异无统计学意义(P>0.05)。无低血糖组患者饮酒后MDA水平低于饮水后(P<0.05);低血糖组患者饮酒后MDA水平高于饮水后(P<0.05)。结论随餐适量饮用白酒对胰岛素治疗的2型糖尿病患者的氧化应激的急性影响随血糖水平而异,在不发生低血糖的前提下,适量饮用白酒可能有抗氧化应激作用;而对于容易发生低血糖患者则不建议饮酒,即使少量饮酒也可能会加重氧化应激反应。Objective To investigate the acute effects of moderate alcohol intake on oxidative stress and inflammation in patients with type 2 diabetes mellitus who receive insulin treatment. Methods Enrolled 40 type 2 diabetes mellitus patients who accorded with inclusion and exclusion criteria and were admitted into the Department of Endocrinology of the First Hospital of Nanjing from May 2012 to August 2013. The patients began to receive insulin pump treatment at admission. Equal amount of mineral water and white spirit were drank with dinner at 17: 30 respectively on day 1 and day 2 after alcoholic consumption,and MDA and s ICAM-1 levels were detected on the next day respectively. Based on the results of continuous glucose monitoring and whether hypoglycemia occurred 24 hours after alcohol intake,the patients were divided into non- hypoglycemia group( n = 25)and hypoglycemia group( n = 15). Results No significant differences were noted between the MDA and s ICAM-1 levels after water drinking and those after alcohol drinking( P〉0. 05). The hypoglycemia group was lower than non- hypoglycemia group in MDA level after water drinking( P〈0. 05); there were no significant differences in MDA level after alcohol drinking,s ICAM-1level after water drinking and s ICAM-1 level after alcohol drinking between hypoglycemia group and non- hypoglycemia group( P〈0. 05). Non- hypoglycemia group had lower MDA level after alcohol drinking than that after water drinking( P〈0. 05);hypoglycemia group had higher MDA level after alcohol drinking than that after water drinking( P〈0. 05). Conclusion The acute effects of moderate alcohol intake on oxidative stress and inflammation in patients with type 2 diabetes mellitus who receive insulin treatment vary with blood glucose level. Without hypoglycemia,moderate intake of white spirit may counter oxidative stress and inflammation,while patients with hypoglycemia are not recommended to drink alcohol,for even a small amount of alcohol intake could aggregate respo
