乙肝病毒对慢性乙肝患者代谢参数的关联性分析

摘　　要：目的探讨慢性乙型肝炎（CHB）患者病毒相关因素对代谢参数的影响。方法分析110例CHB患者不同乙肝病毒（HBV）携带年限组、不同HBV DNA病毒载量组各项代谢参数的差异。结果 1）胰岛素抵抗指数（HOMA-IR）及空腹血糖（FPG）随着HBV携带年限延长而升高,差异有统计学意义（P〈0.05）;2）与其他HBV DNA水平组比较,HBV DNA≥108 uIU/mL组总胆固醇（TC）和低密度脂蛋白胆固醇（LDL-c）水平更高,HBV DNA在10~3~10~5组肝脂肪化比例更高,差异有统计学意义（P〈0.05）。结论 CHB患者HBV携带年限、病毒载量均对代谢参数有影响：HBV携带年限越长越易导致胰岛素抵抗及糖代谢异常;病毒载量高易导致脂代谢异常及肝脂肪化。Objective To analyze virus influencing metabolism parameter in chronic hepatitis B（CHB）.Methods Using the prospective cross-section research.To analyze the difference of each metabolism parameter in different hepatitis B virus carry duration group,different HBV DNA virus load group in 110 cases with CHB.Results 1）Compared with hepatitis B virus carry duration shorter than 5year group and 5~10year group,longer10 year group was older,with higher HOMA-IR and fasting blood glucose.There was statistic significant difference（P〈0.05）.2）Compared with HBV DNA virus load lower than 103,from 103 to 105,from 105 to 108 three groups,higher than 108 group had higher TC and LDL-c.Compares with other three groups,HBV DNA virus load from 103 to 105 group had higher proportion fatty liver.There was statistic significant difference（P〈0.05）.Conclusion The chronic hepatitis B patient hepatitis B virus carry duration,viral load can influent to metabolism parameter.The hepatitis B virus carry duration is longer,insulin resistance and glucose metabolism abnormal is commoner.The viral load is higher,the lipid metabolism abnormal and fatty liver is commoner.
