踝关节骨折合并下胫腓联合损伤29例疗效分析

摘　　要：目的探讨踝关节骨折合并下胫腓联合损伤的诊断、手术技巧和疗效。方法回顾分析我院2008年1月-2014年12月经手术治疗的29例踝关节骨折合并下胫腓联合损伤患者的临床资料。其中男性19例,女性10例,年龄22-70岁,平均年龄（44.1±14.8）岁。结果本组病例随访时间8-62个月,平均随访时间19.7个月,根据Baird-Jackson踝关节评分系统进行疗效评价,其中优16例,良10例,可2例,差1例,优良率89.7%。结论术中行Hook试验评估下胫腓联合的稳定性,解剖复位并选择合理固定方式是保证疗效的关键。Objective To investigate the diagnosis, operative technique and effect of surgical treatment of ankle fracture associated with syndesmotic injury. Methods From January 2008 to December 2014, 29 consecutive adult patients with ankle fracture associated with syndesmotic injury were surgically treated at our institution. 19 male and 10 female patients aged from 22 to 70 years, average（44.1±14.8） years old were analyzed retrospectively. Results All patients were followed-up for 8 to 62 months, average 19.7 months. According Baird-Jackson score, the outcome is 16 excellent, 10 good, 2 fair and 1 poor. The rate of excellent and good is 89.7%. Conclusion Application intraoperative Hook test to assess the stabilization of syndesmosis, anatomic reduction and stable fixation are important for ensuring good outcome.
