摘 要:目的观察孟鲁司特钠对支气管扩张症患者IL-17和IL-10表达的的影响,并分析了其临床疗效。方法收集2013年2月~2015年1月潮州市潮安区庵埠华侨医院治疗的NCFB患者82例,患者随机分为常规治疗对照组和孟鲁司特钠治疗组,每组41例。ELISA法测定痰液中IL-17、IL-10、IL-6和TNF-α表达水平。比较两组肺功能指标、临床症状评分和急性加重次数。结果治疗后两组IL-17、IL-6和TNF-α表达水平均出现减低和IL-10表达水平均出现升高(P〈0.05和P〈0.01),但治疗组IL-17、IL-6和TNF-α表达水平显著低于对照组(P〈0.05),IL-10表达水平又显著高于对照组(P〈0.05)。治疗后两组FEV1%和FEV1/FVC肺功能指标均出现升高(P〈0.05和P〈0.01),但治疗组FEV1%和FEV1/FVC均显著高于对照组(P〈0.05)。治疗后两组咳嗽程度评分和咳痰量评分均减低(P〈0.05和P〈0.01),但治疗组咳嗽程度评分和咳痰量评分均显著低于对照组(P〈0.05)。治疗组治疗期间急性加重1.82±0.35次,显著低于对照组的2.76±0.54次(P〈0.05)。结论孟鲁司特钠可以调节支气管扩张症患者IL-17和IL-10因子的体内平衡,抑制气道局部高炎症反应,保护患者肺功能损伤,减少急性加重次数,临床疗效显著,具有一定的临床应用价值。Objective To detect the effect of Montelukast on IL-17 and IL-10 expression in patients with bronchiectasis and explore its clinical efficacy. Methods A total of 82 patients were enrolled in this study, and they were randomly divided into the control group (n = 41 ) and the montelukast treatment group (n = 41 ). The expression of IL-17, IL-10, IL-6, and TNF-α were detected by ELISA analysis. The pulmonary function, clinical symptom score and the number of acute exacerbation of the two groups were compared. Results After treatment, IL-17, IL- 6, and TNF-α decreased and IL-10 increased in the two groups (P 〈0. 05 or P 〈0. 01 ). However, the improvement was more pronounced in the treatment group than in the control group (P 〈0. 05). FEV1% and FEV1/FVC was obviously higher in the treatment group than in the control group (P 〈 0. 05). The cough severity score and sputum score was lower in the treatment group than in the control group (P 〈 0. 05). During the treatment, the number of acute exacerbation was 1.82 ± 0. 35 times in the treatment group, which was significantly lower than that in the control group (2. 76 ± 0. 54 times) (P 〈 0. 05 ). Conclusion Montelukast can regulate the balance of IL-17 and IL-10 in patients with bronchiectasis, inhibit airway local inflammatory response, protect pulmonary function, and reduce the number of acute exacerbation.
