集束化干预策略对控制基层综合医院儿科院内感染的效果分析

摘　　要：目的:观察集束化干预措施对控制基层综合医院儿科院内感染的效果。方法:整群选取本地区3家基层医院(佛山市南海区第二人民医院、南海区第四人民医院及南海区罗村医院)2014年10月-2015年6月收治的儿科6258例住院患者作为研究对象,对其实施院内感染集束化防控措施,对比干预前后医院感染率、医务人员手卫生依从性与正确率、抗生素使用合格率、平均住院时间与出院费用的变化情况。结果:实施集束化干预后,儿科医院感染率从干预前的2.46%下降为干预后的1.52%,医务人员手卫生依从性与正确率从干预前的48.02%、42.64%上升为干预后的86.25%、78.31%,抗生素使用合格率从干预前的84.23%上升为干预后的94.27%,患者平均住院时间由干预前的(4.51±1.02)d缩短为干预后的(4.38±0.98)d,平均费用从(1499±317)元减少为(1249±308)元。干预前后各项比较差异均具有统计学意义(P<0.05)。结论:对基层综合医院儿科实施集束化医院感染防控干预可有效降低儿科医院感染发生率,提高医务人员手卫生执行率与抗生素使用合格率,同时在有效缩短患儿住院时间和减少住院费用方面也具有显著效果。Objective：To explore the effect of bundles of care in control the nosocomial infections in pediatrics of grassroots general hospital.Method：6258 pediatric inpatients from three basic general hospitals from October 2014 to June 2015 were selected as the research objects and implemented by bundles of care for prevention and control measures of hospital infection, and the nosocomial rate, hand hygiene compliance and correct rate, antibiotic use qualified rate, average hospitalization time and expenses before and after intervention were compared.Result：After performing cluster intervention,the nosocomial infection rate in the department of pediatrics reduced from 2.46% to 1.52%,the hand hygiene compliance and correct rate respectively increased from 48.02%,42.64% to 86.25%,78.31%,the antibiotic use qualified rate increased from 84.23% to 94.27%,the average hospitalization time shortened from（4.51±1.02） days to（4.38±0.98）days and the average expenses reduced from（1499±317） yuan to（1249±308）yuan,the differences in the indexes before and after intervention were statistically significant（P〈0.05）.Conclusion：Bundles of care to pediatrics of grassroots general hospital can reduce the nosocomial rate in pediatrics,improve the hand hygiene compliance and correct rate of medical staff,at the same time,it can effectively shorten average hospitalization time and reduce expenses.
