摘　　要：目的探讨妊娠期糖尿病（GDM）和新生儿体脂含量的关系。方法采用前瞻性队列研究,将孕早期孕妇纳入队列,随访至胎儿出生。于孕24~28周,进行口服葡萄糖耐量试验（OGTT）,将研究对象分为正常组和GDM组。孕妇分娩前,留取空腹血;胎儿出生24小时内,进行体格测量。使用t检验比较新生儿体脂质量、体脂百分含量之间的差异;多元线性回归分析GDM和新生儿体脂含量之间的关系。结果 GDM组新生儿体脂质量（0.62±0.27）kg、体脂百分含量（17.68±5.22）%均高于正常组,差异具有统计学意义（P〈0.001）。多元线性回归结果表明分娩孕周、孕前体质指数（BMI）、空腹血糖、血脂水平是新生儿体脂百分含量的危险因素（P〈0.05）。结论 GDM增加新生儿的体脂含量,应早发现、早干预,以降低新生儿体脂含量。Objective To investigate the association between gestational diabetes mellitus（ GDM） and percentage of neonatal body fat. Methods A prospective cohort study was conducted including those women who were during early pregnancy. We followed up the women until fetal births. All the women underwent a 75- g,2- h oral glucose tolerance test（ OGTT） during 24~ 28 gestational weeks. The research subjects were divided into two groups,normal group,and GDM group. Maternal blood samples were obtained before fetal births and anthropometric measurements were performed within 24 hours of fetal births. The differences in anthropometric measurements between two groups were tested by t- test. Multiple linear regressions were used to examine the association between GDM and percentage of neonatal body fat. Results The mean of fat mass（ 0. 62 ± 0. 27） kg and percentage of body fat（ 17. 68 ± 5. 22） % in offspring born to mothers with GDM were higher than normal group. The differences were statistically significant（ P 〈0. 001）. The results of multiple linear regression showed that gestational age at delivery,maternal pre- pregnancy body mass index（ BMI）,fasting plasma glucose and triglyceride levels were associated with the percentage of neonatal body fat（ P〈 0. 05）. Conclusion GDM could increase the percentage of neonatal body fat. Early discovery and early intervention should be made to reduce neonatal body fat.
