摘  要:目的了解健康人群肠道病毒动态变化及手足口病肠道病毒型别,探讨健康人群肠道病毒在手足口病传播过程中的作用。方法采用多阶段整群抽样的方法,分别于手足口病流行前期(3-4月)、流行高峰期(6-7月)、流行后期(10-11月)采集健康人群及手足口病病例粪便标本,采用荧光定量PCR进行肠道病毒核酸检测。结果 3次共采集健康人群粪便标本1235份,肠道病毒通用引物阳性177份,阳性率为14.33%,检出COAX16型肠道病毒1株,非EV71非Cox A16其它型肠道病毒176株;手足口病病原学以EV71和Cox A16型为主,非EV71非Cox A16其他肠道病毒仅占26.98%。不同时期、不同年龄组带毒率有统计学意义(P〈0.05)。〈1岁组健康人群肠道病毒阳性率最高,而发病率最低,呈相反趋势;1~组发病率最高,从1~组呈相同趋势。健康人群肠道病毒阳性率与发病率无相关性(P〉0.05)。结论健康人群以携带非EV71非Cox A16肠道病毒为主,健康人群肠道病毒带毒在手足口病传播意义不大。Objective To understand the dynamic changes of enterovirus and hand- foot- mouth disease in the healthy population,and to investigate the relationship between the enterovirus in healthy population and hand- foot- mouth. Methods Multistage stratified cluster sampling method was adopted in this study,and all specimens were collected randomly in the early epidemic season( March- April),epidemic season( June- July) and post- epidemic season( October- November).Real- time PCR was used to isolate and identify of the enterovirus. Results 1235 specimens of the healthy population were collected. 177 specimens were positive with enterovirus,with the positive rate 14. 33%. 176 cases were identified with other enterovirus than EV71 or Cox A16. Only one specimen was found positive with a Cox A16 subtype,and no EV71 subtype was detected. EV71 and Cox A16 were the major cause of HFMD,and other types of enterovirus( non- EV71 and non- Cox A16)accounted for 26. 98%. There was statistically significant difference in the prevalence at different times and different ages.Children under 1- year- old had the highest rate of enterovirus infection,but contrastingly the incidence rate was the lowest.Children older than 1 year had the highest rate of incidence. There was no relationship between the rate of infection and incidence. Conclusion The healthy population mainly carried the enterovirus of non- EV71 and non- Cox A16,and the infection rate has no relation to the rate of incidence.
