摘 要:目的研究血液透析液和透析用水中细菌及革兰阴性菌的污染状况。方法采用改良胰酪胨大豆酵母浸膏琼脂(TSA)平板涂布法定量测定血液透析液和透析用水中革兰阴性污染菌数量、VITEK2鉴定仪进行菌种鉴定,营养琼脂倾注法检测血液透析液和透析用水中污染菌菌落总数。结果 17份血液透析用水污染菌平均菌落总数为19cfu/ml,革兰阴性污染菌的平均菌落数为7cfu/ml,61份入口处血液透析液污染菌平均菌落总数为122cfu/ml,其中革兰阴性污染菌平均菌落数为54cfu/ml;61份出口处血液透析液污染菌平均菌落总数为307cfu/ml,其中革兰阴性污染菌平均菌落数为174cfu/ml,透析液革兰阴性污染菌和污染菌菌落总数入口处、出口处均存在显著性差异(P<0.05)。本文检出的革兰阴性污染菌主要有普罗威登菌、肠杆菌、黄杆菌、产碱杆菌、假单胞菌和不动杆菌属等机会致病菌。结论血液透析液有一定程度革兰阴性菌污染,透析用水和透析液存在操作污染或系统自身污染。Objective To study the types of bacteria and the gram- negative contamination situation in the hemodialysis liquid and dialysis water. Methods The improved TSA spread plate method was adopted to detect the numbers of gram- negative contamination bacteria in the hemodialysis liquid and dialysis water,and VITEK2 Compact was used to identify the types of bacteria. Results Among the 17 hemodialysis liquid samples,the average number of colonies of the contamination bacteria and Gram- negative contamination bacteria was 19 cfu / ml and 7cfu / ml,respectively. In the entrance of the 61 hemodialysis liquid samples,the average number of colonies of the contamination bacteria and Gram- negative contamination bacteria was 122 cfu /ml and 54 cfu / ml,respectively. At the exit was 307 cfu / ml and 174 cfu / ml. The detected Gram- negative contamination bacteria consist of Providencia,Enterobacter Hormaeche and Edwards,Flavobacterium columnar,Alcaligenes Castellani Chalmers,P.Aeruginosa,and Acinetobacter. Conclusion The hemodialysis liquid had a certain degree of the Gram- negative bacteria contamination. The dialysis water was polluted by operation or the system itself.
