摘  要:目的探讨多轴锁定钢板治疗老年Jensen-EvansⅤ型股骨转子间骨折的临床疗效。方法应用股骨远端多轴锁定钢板倒置治疗31例老年Jensen-EvansⅤ型股骨转子间骨折患者。结果患者均获得随访,时间16~36(27.4±6.5)个月。手术时间为38~72(50±8.2)min,C臂机透视次数为3~8(5.3±2.5)次,术中失血量为100~220(108±10.5)ml,骨折愈合时间为13~18(12.2±5.5)周。1例出现骨折内固定失效,二期给予髋关节置换;2例出现股骨头坏死,二期给予髋关节置换;3例出现转子部位屈髋痛,钢板取出后疼痛消失。根据Harris髋关节功能评分:优16例,良9例,差6例。结论多轴锁定钢板治疗老年Jensen-EvansⅤ型股骨转子间骨折手术时间短,C臂机透视次数少,失血量少,固定可靠。Objective To study clinical effects of polyaxial locked-plate for the treatment of Jensen-Evans type Ⅴfemoral intertrochanteric fractures. Methods 31 patients with Jensen-Evans type Ⅴ femoral intertrochanteric fractures were treated by reverse polyaxial locked-plate. Results All patients were followed up for 16 ~ 36( 27. 4 ±6. 5) months,the operation time was 38 ~ 72( 50 ± 8. 2) min,the fluoroscopy times were 3 ~ 8( 5. 3 ± 2. 5) times,the bleed losing was 100 ~ 220( 108 ± 10. 5) ml,the healing time of fracture was 13 ~ 18( 12. 2 ± 5. 5) weeks. One case received the arthroplasty because of the failure of implant; and 2 cases received the arthroplasty because of femoral head necrosis; 3 cases complain of flexion of the hip pain and the pain was dismissed after the plates were removed. The clinical effect were judged by Harris scoring criterion,16 cases had excellent effect,9 good and 6 poor.Conclusions The polyaxial locking plate for the treatment of Jensen-Evans type Ⅴ femoral intertrochanteric fractures,which has advantanges including shorted operated time,reliable fixation,less fluoroscopy times and blood losing.
